Abstract
Purpose
To assess the quality of anticoagulation control at an out‐patient clinic and to investigate patterns of deviation from therapeutic ranges.
Design/methodology/approach
Records for 36,157 clinic visits (2,050 patients) were studied. The quality of anticoagulation control was assessed by comparing the measurement of pro‐thrombin time recorded at each clinic visit, expressed as an international normalised ratio (INR), with the target therapeutic range for that patient, also recorded at the time of the clinic visit. Each INR measurement was classified according to the relevant patient's therapeutic range and the signed difference between the INR measurement and the centre of the therapeutic range was calculated. For each patient the percentage of their INR measurements that lay within their therapeutic range was calculated.
Findings
Of the measurements, 52.3 per cent were within the relevant therapeutic range. The proportion of individual patients' INR measurements within range varied greatly (median 52 per cent, inter‐quartile range 40‐65 per cent). The quality of anticoagulation control, as measured by the proportion of patients within their therapeutic range, changed little with patient follow‐up time.
Originality/value
The quality of anticoagulation control reported is comparable with that at other centres. The vast majority of patients spend periods outside the therapeutic range for their condition. There may be considerable room for improvement.

In this commentary, I highlight a few of the assertions made by McDaniel et al. (2013) about the importance of complexity science guided management practices, and extend these ideas specifically to how we might think about reducing seemingly intractable problems in health care such as patient safety, patient falls, hospital acquired infection, and the rise of chronic illness and obesity. I suggest that such changes will require managers and providers to view health care organizations and patients as complex adaptive systems and include patients as full participants in co-producing their health care.

For many decades, foreign correspondents have been regarded as a highly prestigious press corps with the core proposition of bearing witness to events in remote places. However, the advent of the Internet and the new technologies has challenged this position. Citizens living where the events occur can make use of a wide range of digital technologies and inform the rest of the world, without the need for the journalist intermediaries who were essential in the past. In addition, the new economic pressures brought to legacy media by the digital technology have paved the way for the rise of a new type of foreign correspondent, the multiskilled staffer, who has to be technologically literate in order to fulfil his daily task. This study based on 51 interviews with foreign correspondents aims at investigating how the foreign correspondents perceive these trends in their daily working routines and if the digital technology has caused a deprofessionalization of the foreign correspondence or we are witnessing the emergence of a new professional discourse which embraces a new core of professional traits.

Abstract
Young children under the age of five are particularly overrepresented in traumatic brain injury (TBI) due to accidents and falls. To remediate the problems, confronting young children with TBI, is critical that they are introduced to opportunities to be placed in general education classrooms at the earliest possible point. The purposes of this chapter are to (1) describe causes, symptoms, and challenges following TBI (e.g., physical, emotional, and cognitive difficulties), (2) distinguish mild TBI (MTBI) from other mild categories of disability, (3) identify classroom interventions and strategies, and (4) identify parenting strategies that may provide essential support for them in adjusting to and managing their young child’s difficulties.

Abstract
We reviewed three existing reviews of literature: two related to cultural and linguistic diversity in well-regarded special education research outlets including Advances in Special Education, and the third regarding constructions of culture, race, disability, and risk in early childhood and early childhood special education (ECSE) literature. Some of our findings reflected ongoing oppressions for young children at the intersections of race, disability, and other forms of social difference to which negative treatment has been attached, including static and deficit-based framings of disability, reliance on whiteness, and English as the norm for developmental benchmarks, and failure to account for disability beyond medical models. We present a preliminary framework for special education research and practice considerations in order to remediate these issues in ECSE for young learners of color, among others, with disabilities.

Abstract
The chapter focuses on humanizing higher education by infusing ethical leadership in the curriculum to improve the public service. Its design is qualitative in nature and literature reviews and document analysis were employed in compiling the chapter. It followed an interpretive paradigm and used Kolb’s Experiential Learning Theory as a lens in understanding humanizing education in higher education. Nowadays ethical leadership is of paramount importance in higher education and in the public service. Ethical leadership should be based on the moral person and on the moral manager. The moral person component focuses on desirable personal qualities of leaders such as being perceived as honest, fair and trustworthy. The moral manager focuses on the leader and uses transactional efforts such as rewards and punishments to reinforce desired behaviors. Soft skills are very important in higher education and should be transferred through coursework. Students need to be supported in all aspects of education including the academic, emotional and social demands in higher education.

Abstract
Historically, the condition we now refer to as intellectual disability has been conceptualized using models that were extension of the medical model. Recent advances, however, have emphasized person-environment fit models of disability that view disability, intellectual, and other cognitive disabilities, as the lack of fit between a person’s capacities and the demands of the context. This chapter examines these shifts in conceptualization and the ways in which this changes how interventions are designed to provide support to enable people with intellectual disability to live, learn, work, and play in their communities. Such interventions and supports include issues pertaining to Universal Design for Learning, multi-tiered systems of supports, and the primacy of promoting the self-determination of people with disabilities. The importance of efforts to promote social inclusion is also discussed, as well as strategies to promote transition to adulthood. Authors from several countries provide examples of how these new intervention paradigms are being implemented across the world.

Abstract
The increasing economic importance and the number of born global firms make it worthwhile to study what leads to their success in the international market. To better understand this international business phenomenon, we conducted in‐depth interviews with managers, coupled with public database and Web site searches. Research propositions were developed based on an extensive qualitative method. The relationship between organizational culture, information technology capability, and performance is proposed in the context of born global firms, based on viewing the concept of IT capability from the resource‐based view. We further provide recommendations for managers, theoretical contributions and suggestions for future research.

Abstract
Public procurement is one of the principal instruments used by the Commission to open up the European Single Market. This paper presents a critical assessment of public procurement policy in the context of developing policies on electronic commerce and the Government Procurement Agreement (GPA) within the World Trade Organisation (WTO). The paper focuses on how business can access public procurement opportunities in Europe, and what opportunities exist for improved electronic means of access to information. The proposed future for electronic tendering in Europe, SIMAP, is discussed and compared to similar systems in the USA. The paper suggests that a new legislative framework is required for public procurement and electronic commerce in Europe to ensure that governments and businesses do not suffer a competitive disadvantage in the electronic future of world trade.
