The principal focus of clinical governance is intended to be at the level of the statutory organisation such as a Health Authority, Primary Care Trust or NHS Trust. This paper suggests that there are at least three levels (micro, macro and meta) at which clinical governance must operate if the original vision of flourishing excellence is to be fulfilled. These do not, regrettably, fit comfortably into the current organisational arrangements for the NHS in England and some cut across any traditional understanding of accountability relationships. With clinical governance at a relatively early stage of its development in many hospitals, and little better than vestigial in most Primary Care and Ambulance Trusts, it may be premature to take on such complex and challenging issues. However, the types of scenario described are typical of those that might create the next major system failure if they remain beyond the scope of clinical governance.

The daily loaf or cake of bread is undoubtedly Man's first and oldest form of prepared food. Except in the the cereal used to make it, it differs little the world all over, but changes are taking place in this staple food, in its preparation and distribution. In recent legal proceedings, a charge of selling a loaf not of the nature, etc., in that it contained rodent contamination, was brought against partners in what was described as the last surviving family baker's business in a relatively large and populous suburban area of London. “Take‐over bids” are the order of the day and in modern business, which includes the baking of bread, the accent is on combination and concentration. This chapter provides an overview of the findings and chapters of a thematic volume in the International Perspectives on Education and Society (IPES) series. It describes the common dataset and methods used by an international research team.

The chapter synthesizes the results of a series of country-level case studies and cross-national and regional comparisons on the growth of scientific research from 1900 until 2011. Additionally, the chapter provides a quantitative analysis of global trends in scientific, peer-reviewed publishing over the same period.

The introduction identifies common themes that emerged across the case studies examined in-depth during the multi-year research project Science Productivity, Higher Education, Research and Development and the Knowledge Society (SPHERE). First, universities have long been and are increasingly the primary organizations in science production around the globe. Second, the chapters describe in-country and cross-country patterns of competition and collaboration in scientific publications. Third, the chapters describe the national policy environments and institutionalized organizational forms that foster scientific research.

The introduction reviews selected findings and limitations of previous bibliometric studies and explains that the chapters in the volume address these limitations by applying neo-institutional theoretical frameworks to analyze bibliometric data over an extensive period.

Explores the causes, outcomes and remedies of cladding failure in its many forms, from life threatening component separation and glass breakage, to costly water intrusion and air infiltration problems. Demonstrates the cumulative effect of errors commonly made during the specification, design and installation stages of a cladding system through a case study of a 26 storey high‐rise casino hotel in Atlantic City, New Jersey. Outlines the project details, repair programme and resulting legal action taken. Suggests that as cladding systems and materials continue to develop the likelihood of failure will continue to increase.

The purpose of this paper is to outline a work‐based, professional education series of workshops, focusing on issues of sex and sexuality training for health and social care professionals who work with adults with a moderate to severe acquired brain injury. The series of workshops were conducted by a trained psychosexual therapist and couples counsellor from Headway Brain Injury Association, and held at the Royal Hospital for Neuro‐disability in 2009 to raise health and social care professionals' awareness about sexuality issues for this patient population. A case study of the series is presented, which we hope will add to the discourse about the sexual needs of people with a moderate to severe acquired brain injury. Overall, participants were positive about the education and learning achieved within the workshop series.

Discusses Marks & Spencer′s application of computer‐based training (CBT) in training its food supervisors. CBT is used to enhance the transfer of learning following training input using workbooks. Argues that the particular application is a very powerful and effective use of CBT, illustrating the real potential of CBT when integrated with other training techniques. Reflects on how such an application may assist in the development of CBT more generally.

An important step towards ousting the Germans from a lucrative branch of West African trade in which Germany has hitherto held almost a monopoly has been proposed by a Colonial Office Committee and adopted by the Government. This Committee was appointed a year ago by Mr. BONAR LAW, with Mr. STEEL‐MAIT‐LAND, M.P., as chairman, “to consider and report upon the present condition and the prospects of the West African trade in palm kernels and other edible and oil‐producing nuts and seeds, and to make recommendations for the promotion in the United Kingdom of the industries dependent thereon.”
