Abstract
Purpose
The purpose of this paper is to present an evaluation of a collaborative commissioning approach to improve quality and experience and reduce cost within integrated health and social care.
Design/methodology/approach
A multi-method approach is used involving qualitative interviews, documentary analysis and non-participant observation.
Findings
The findings suggest that the approach provides a suitable framework for the collaborative commissioning of integrated health and social care services.
Research limitations/implications
Further research is now needed to provide a definitive evaluation of its value outside of Wales.
Practical implications
With the significant scrutiny on health systems, the approach demonstrates effectiveness in securing quality improvements, achievement of recognised care standards and patient outcomes, while providing scope for financial gains and a goal for stakeholders to engage in effective communication.
Originality/value
This research presents an innovative method for collaborative commissioning and reveals activities that appear to contribute to more effective commissioning processes.

Abstract
Purpose
The teaching of cooking is an important aspect of school-based efforts to promote healthy diets among children, and is frequently done by external agencies. Within a limited evidence base relating to cooking interventions in schools, there are important questions about how interventions are integrated within school settings. The purpose of this paper is to examine how a mobile classroom (Cooking Bus) sought to strengthen connections between schools and cooking, and drawing on the concept of the sociotechnical network, theorise the interactions between the Bus and school contexts.
Design/methodology/approach
Methods comprised a postal questionnaire to 76 schools which had received a Bus visit, and case studies of the Bus’ work in five schools, including a range of school sizes and urban/rural locations. Case studies comprised observation of Cooking Bus sessions, and interviews with school staff.
Findings
The Cooking Bus forged connections with schools through aligning intervention and schools’ goals, focussing on pupils’ cooking skills, training teachers and contributing to schools’ existing cooking-related activities. The Bus expanded its sociotechnical network through post-visit integration of cooking activities within schools, particularly teachers’ use of intervention cooking kits.
Research limitations/implications
The paper highlights the need for research on the long-term impacts of school cooking interventions, and better understanding of the interaction between interventions and school contexts.
Originality/value
This paper adds to the limited evidence base on school-based cooking interventions by theorising how cooking interventions relate to school settings, and how they may achieve integration.

Abstract
The manager must take into account various human factors when introducing computers into the library. A survey of literature reveals the key fears of employees involved in library automation — to be concerned with job security, job satisfaction and health and safety. These findings are compared with data collected from a questionnaire administered to staff in four libraries, all in the process of automating various tasks. The effects of computerisation of the library service on users is also examined, with an analysis of literature and a questionnaire sent to 30 students in the College for the Distributive Trades. The author discusses ways in which the manager can interest and motivate staff by eliminating causes of dissatisfaction and by taking positive steps in appealing to employees' self‐interest in the possibilities of career advancement and the challenge of mastering something new. Staff selection procedures need to be amended to acquire the necessary skills, and training should be ongoing. The response of users to library automation is often enthusiastic but real benefits can be difficult to measure. The onus is ultimately on librarians to demonstrate that they still have a key role to play in the provision of information.

Abstract
Purpose
UK fellowship schemes have been set up to address low-level engagement of doctors with leadership roles. Established in 2013, the Welsh Clinical Leadership Fellowship (WCLF) programme aims to recruit aspiring future clinical leaders and equip them with knowledge and skills to lead improvements in healthcare delivery. This paper aims to evaluate the 12-month WCLF programme in its first two years of operation.
Design/methodology/approach
Focused on the participants (n = 8), the authors explored expectations of the programme, reactions to academic components (provided by Academi Wales) and learning from workplace projects and other opportunities. The authors adopted a qualitative approach, collecting data from four focus groups, 20 individual face-to-face or telephone interviews with fellows and project supervisors and observation of Academi Wales training days.
Findings
Although from diverse specialties and stages in training, all participants reported that the Fellowship met expectations. Fellows learned leadership theory, developing understanding of leadership and teamwork in complex organisations. Through workplace projects, they applied their knowledge, learning from both success and failure. The quality of communication with fellows distinguished the better supervisors and impacted on project success.
Research limitations/implications
Small participant numbers limit generalisability. The authors did not evaluate longer-term impact.
Practical implications
Doctors are required to be both clinically proficient and influence service delivery and improve patient care. The WCLF programme addresses both the need for leadership theory (through the Academi Wales training) and the application of learning through the performance of leadership roles in the projects.
Originality/value
This work represents an evaluation of the only leadership programme in Wales, and outcomes have led to improvements.

Abstract
Purpose
It is well recognised that individuals have much to contribute to the care that they receive, with attendant benefits on outcomes and reduction in cost. The recognition of individuals who access care services as interdependent citizens embedded in both formal and informal support networks is a shift that acknowledges their active role as partners in management of their own care and in service innovation and development. The purpose of this paper is therefore to explore and illustrate some of the domains of co-production.
Design/methodology/approach
In this paper, the authors review the literature, both peer-reviewed and professional, in order to provide a broad and contemporary commentary on this emergent approach. This literature is critically summarised and presented along with a narrative that discusses the context in Wales, where the authors are based. The approach to this paper is to bring together existing knowledge and also propose potential avenues for further research and practise development.
Findings
There is a diverse literature on this topic and the application of co-production appears potentially transformational within health and social care. Implementation of the principles of co-production has the potential to improve health and social care services in a range of settings. Real changes in outcomes and experience and reduction in societal cost can be achieved by making the people of Wales active partners in the design and delivery of their own health and social care.
Originality/value
This review offers a readily accessible commentary on co-production, which may be of value to a wide range of professional groups and policy makers. This paper also reflects an original attempt to summarise knowledge and propose further areas for work. Most importantly, this paper offers a start point for co-production to become a reality for service provision with all the attendant benefits that will arise from this development.
