Abstract
Describes the results of a postal questionnaire survey of all 1,383 hospital consultants in the North Western Region of the UK in 1994; updating a similar survey conducted in 1987. In both surveys, consultants were asked to describe their current management role, management training received and any perceived future training needs. A series of open questions in the 1994 survey explored barriers and incentives to the take‐up of management training. The results show that in 1994 more doctors were taking on greater management responsibility and from an earlier age. Consequently, the proportion of consultants expressing a need for management training had risen from 62 per cent in 1987 to 73 per cent in 1994. The most useful courses were local budgeting and business planning. However, many consultants described problems in accessing training. Concludes by highlighting policy implications arising from the surveys which will need to be addressed if consultants are to fulfil their management potential.

In the last issue of VINE we had a look at the serials control package — ISIS from Blackwell's. As a contrast we now take a look at a bespoke serials check‐in system developed at The British Library Document Supply Centre (BLDSC) at Boston Spa. The BLDSC has somewhat unique requirements from a serials check‐in system. The library currently subscribes to 56,000 journal titles and receives approximately 350,000 issues each year arriving at a rate of 1,500 issues per day. These are handled in the Accessions Department by a team of 8 who aim to have the issues checked in fast enough to be available on the shelves by 4.30pm each day. To help to achieve this aim SACHET (Serials Accessions CHEck‐in of Titles) was devised and developed in‐house by the BLDSC Computing Department.

Abstract
Blockchain is an innovative and emerging technology that allows the structuring of open data in a secure and decentralized manner. This technology, coupled with artificial intelligence (AI), is a driver of digital transformation that permeates all sectors of the economy. In this context, it becomes relevant to understand the applicability of AI and blockchain in education, more specifically their use in higher education. Thus, the following problem arises: how can blockchain technology and AI be applied to higher education? To answer this problem, the following objective was set: to map by means of a scoping review how AI and blockchain can be applied in education as a technology for higher education. To this end, a bibliometric search was conducted in the Web of Science (WoS) and Scopus databases, adopting the PRiSM methodology for scoping review. The results showed that blockchain and AI can be used in higher education through a smart contract that serves to motivate teamwork and student involvement in activities performed in the school context and also to increase machine reliability by establishing a chain of information. It is also concluded that these technologies will help form a generation ready to work in a more collaborative way, adopting attitudes of sustainability in its local and global context for solving complex problems of everyday life in the 21st century.

Purpose
Lesson study (LS) is a professional development approach that has been attracting attention as an educational innovation since the late 1990s. The purpose of this paper is to investigate factors that contribute to the adoption of LS by schools in developing countries without development assistance.
Design/methodology/approach
The diffusion of innovation theory was used as an explanatory device. The study examined the characteristics of LS perceived by 28 teachers in an Indonesian primary school, where LS was actively and autonomously adopted without development assistance. Data were collected from multiple sources, including interviews with some teachers and a questionnaire for all 28 teachers.
Findings
While previous studies have indicated that LS as an educational innovation lacks the ideal sets of perceived characteristics that could promote its adoption, this study found that such negative characteristics were mitigated in the Indonesian school. It also found that some of the factors facilitating LS adoption may be information on the outcomes of LS and less hierarchical relationships among teachers and professors. Furthermore, active school leadership was found to be a significant factor in this adoption.
Originality/value
Regarding adoption of LS in developing countries, previous studies focused on how development assistance works, what strategies of development assistance are necessary for introducing LS and how development assistance programs can be sustained. However, scant attention has been paid to how schools in developing countries have fared without development assistance. This study sheds light on this missing point.

Abstract
Purpose
Existing treatments for alcohol and substance dependence are of limited efficacy and acceptability to users. Since such dependence is associated with high co‐morbidity and mortality rates, alternative treatments should be urgently evaluated. Ibogaine, a hallucinogenic alkaloid, derived from Tabernanthe Iboga, a plant unique to the rainforests of West Africa, is already associated with a large underground treatment scene. This paper aims to explore this issue.
Design/methodology/approach
The paper draws on the comparative ethnography of magicoreligious Iboga use in West Africa alongside contemporary accounts of ibogaine addiction treatment, and contextualizes these within the neurobiology and anthropology of addiction and treatment provision.
Findings
While the exotic otherness of ibogaine no doubt contributes to interest and hope, the impact of the medicine, not just on craving and withdrawal, but on the perceived acquisition of psycho‐spiritual insight, may validate such anticipation. Both West African and Western Ibogaine use are explored in the context of oppression, resistance and a re‐asserted morality. Successful ibogaine treatments appear to respect but transform existing subcultural meanings and identities, while promoting realism and psychosocial integration.
Practical implications
Existing treatment programmes might learn from the inherent critique of conventional treatment enacted in ibogaine treatment scenes.
Social implications
Lack of mainstream investment may partly reflect the cultural positioning of drug‐users (and of this drug‐class) yet the scale of underground ibogaine use demands urgent funding for research to optimise safety and further understanding/development.
Originality/value
An understanding of how addicts seeking ibogaine are culturally positioned, and position themselves, illuminates the specific attraction of ibogaine treatment for this group, and has important implications for practice, drug policy and treatment development.

In a circular letter, addressed to local authorities by the Board of Agriculture on December 28, 1901, with reference to the Milk Regulations, the Board suggested that in the absence of any special circumstances indicating the commission of fraud, the local authority might in the first instance call the attention of the vendor to the adverse report of the analyst, and afford him an opportunity of submitting any explanation he might desire to offer on the subject. The Board further expressed the opinion that if the explanation were one which the local authority “felt able” to accept, they might, in the exercise of their discretion, refrain from the institution of proceedings, or withdraw any summons which it might have been necessary to take out in order to avoid the failure of proceedings, at the same time making arrangements for the taking of further samples of the milk supplied, in order that a satisfactory conclusion as to its character might be arrived at. The issue of this letter was obviously a retrograde step, which could only be taken to indicate that the Board were “wobbling” over the milk standards—standards laid down by themselves on the strength of the overwhelming evidence in favour of the institution of those standards as absolute minima, which was laid before the Board's Departmental Milk Committee in 1900. If any proof were wanting that this is a correct view of the case, that proof would be afforded by the issue, on March 27 last, of a further circular letter from the Board, in which the views expressed in the former letter are reiterated, and the study of which can only produce amazement, not unmingled with disgust, among those who have had any experience worthy of the name as regards the working of the Adulteration Acts in this country. Presumably the Regulations were laid down upon due and proper cause shown. By issuing the documents referred to the Board have called the validity of their own Regulations in question, and have suggested that public authorities should base no action upon those Regulations in the absence of other evidence, the nature of which is not stated, indicating “the commission of fraud.” The action of the Board amounts to a smack in the face for the producer of honest and genuine milk such as the purchaser is entitled to get, and can only tend to the introduction of additional loopholes of escape for the dishonest and incompetent.
