Abstract
Purpose
The purpose of this paper is to improve surgical antimicrobial prophylaxis (SAP) prescribing in orthopaedic surgery using the model for improvement framework.
Design/methodology/approach
Orthopaedic patients receiving joint replacements, hip fracture repairs or open-reduction internal-fixation procedures were included. Antimicrobial(s); dose, time of administration and duration of SAP were evaluated for appropriateness based on the local SAP guidelines. After baseline data collection, a driver diagram was constructed with interventions devised for plan-do-study-act cycles. Data were fed back weekly using a point prevalence design (PPD). Interventions included SAP guideline changes, reminders and tools to support key messages.
Findings
SAP in 168 orthopaedic surgeries from 15 June 2016 to 31 January 2017 was studied. Prescribing appropriateness improved from 20 to 78 per cent. Junior doctor changeover necessitated additional education and reminders.
Practical implications
Due to constant staff changeover; continuous data collection, communication, education and reminders are essential to ensure continuous compliance with clinical guidance. Patients with hip fractures are difficult to weigh, requiring weight estimation for weight-based antimicrobial dosing. Unintended consequences of interventions included the necessity to change pre-operative workflow to accommodate reconstitution time of additional antimicrobials and inadvertent continuation of new antimicrobials post-operatively.
Originality/value
Rather than perform the traditional retrospective focused audit, we established a prospective, continuous, interventional quality improvement (QI) project focusing on internal processes within the control of the project team with rapid cyclical changes and interventions. The weekly PPD was pragmatic and enabled the QI project to be sustained with no additional resources.

This study draws parallels between the Major and Johnson eras to reclaim a discursive space beyond the media and political battlefields to examine long-term systemic failure of government PR.
Design/methodology/approach
As part of a wider study into government communications from 1979 to date, this paper draws on evidence from government archives from the 1990s, as well as contemporary accounts, official documents, media accounts, memoirs and biographies, to examine the PR record of two Conservative administrations divided by three decades.
Findings
News management during the Major premiership is worth serious scrutiny, not just as an interlude between two media-friendly Prime Ministers, Thatcher and Blair, but in comparison to Boris Johnson's struggle to contain the news narrative between 2019 and 2022. Both administrations experienced terminal reputational crises during their closing years but their means of managing the news were counter-productive and damaging to public trust (65).
Practical implications
Does this failure in public communication illustrate a systemic dysfunction in government-media relations and, if so, what is the role of government PR in these circumstances?
Originality/value
This article uses a comparison between fixed and moving variables associated with two very different administrations to identify the causes of ongoing systemic failure in government communication.

Arts organisations, unsure of the level of continued government funding available and confronted with the need ever to improve, are seeking new ideas upon which they can focus. At a time when leadership and governance in arts organisations have changed in line with cultural expectations, how is their ethical stance assessed? How does their ethical stance impact on reputation? The challenge to build a good reputation starts at the top of the organisation; however, traditionally, one type of arts organisation, art museums, has focused on the activities level. In an age of globalisation, economic restructuring and technological change, museums therefore may be seen as a contradiction. Traditionally seen as temples for the muses, today’s museums are being challenged to be ethical for society and to build their reputation. As a solution, proposes a cooperative model of cultural organisational ethics that attempts to provide a framework by which arts organisations can put in place ethical artefacts that enhance organisational reputation, rather than detract from it.

Abstract
Purpose
Evidence from Australia and worldwide has highlighted the ongoing marginalisation, discrimination, abuse, violence and neglect of people with disability. One of the main areas of concern is that despite such evidence there remain fundamental barriers for people with disability to report violence and/or access supports. Significantly few studies have canvased the perspectives of people with intellectual disability (ID), family members or disability service providers. Accordingly as a first step, the purpose of this paper is to explore the perceptions of disability and violence-response professionals’ regarding the barriers and facilitators of effective support for people with ID reporting sexual violence and accessing mainstream supports.
Design/methodology/approach
A qualitative research design informed the basis of this study. Semi-structured interviews were conducted with seven participants who held roles within disability services and mainstream violence response. Interviews were recorded and transcribed. These transcripts were then analysed individually using a combination of thematic analysis and discourse analysis to bring to light the social and material structures within service systems in terms of how they disempower or empower supports for people with ID.
Findings
Participants from both services reported that professionals (particularly those outside the disability sector) lacked awareness of practical ways in which they could support people with ID to report and access services outside of disability-specific services. Participants also felt the capacity for people with ID to have a voice about what happens in the “aftermath” of reporting sexual violence was rarely acknowledged. Participants called for education and development of resources which could guide professional practice across disability and wider violence-response services. Participants saw investment in building interagency relationships across sectors as key to supporting the citizenship of people with ID.
Originality/value
This study suggests that there is a strong need for better community engagement and understanding of the multi-faceted issues surrounding responses to violence concerning people with ID. The findings of this study also outline implications for practice and policy which may be of interest to professionals both within disability and wider violence-response sectors in Australia and internationally.

Purpose
Labour came to power in 1997 and immediately transferred many features of its party political news management style into government, overseeing the departure of most of the civil service communications leadership within two years, and developing the media management role of politically appointed special advisers. The purpose of this paper is to empirically examine the changes in custom and practice that operated behind the scenes in government communications between 1997 and 2015, asking to what extent such changes challenge public trust in government in a liberal democracy.
Design/methodology/approach
This study takes a longitudinal, qualitative approach through in-depth interviews with former civil servants, journalists and special advisers, together with documentary and archival evidence. The data were analysed thematically through the text-processing software, NVivo.
Findings
The paper finds that although the controversy over the 2002 dossier on Iraq’s Weapons of Mass Destruction was an extraordinary episode, the creative approach to news management that characterised this case still operates within UK Government communications: the bypassing of civil servants, a partial approach to the facts, selective briefing of favoured journalists, a lack of due process in the management of information and a disregard for the letter and spirit of propriety codes. This has implications for public trust and confidence in the workings of liberal democracy.
Originality/value
The study adopts a mediatisation approach to the study of public relations, using the concept of the “cross field” to demonstrate how PR professionals share media management responsibilities with a number of different promotional actors. PR professionals in government must therefore navigate between the hidden, competitive and demanding worlds of politics, the media and bureaucracy, working with journalists, politicians and political operatives to craft the narratives that seek to drive public opinion.

Purpose
The purpose of this paper is to show how early planned PR efforts at the British Family Planning Association [FPA] resulted in an epoch-making television appearance in November 1955, tessellating with current methodological debates in the history of PR.
Design/methodology/approach
This paper uses a qualitative, micro-history approach and original archival document research conducted at Wellcome Collection, London and the BBC Written Archives Centre, Caversham, to reconstruct early PR activity at the FPA. It intercedes in debates on historiography, the diversification of the history of PR and the concepts of mediatization and advocacy in historical contexts.
Findings
Attaining broadcast coverage for birth control issues was historically difficult and was made more so by Marie Stopes. The subject was commonly packaged into the less problematic issues of population and infertility. The FPA achieved explicit television coverage in 1955 after establishing a focussed PR plan to stage and exploit a silver jubilee event. This vindicated the FPA's mission, validated service users and created broadcast opportunities.
Research limitations/implications
Research is limited by temporal scope (1870s–1950s), and reliance on document sources, footage of television programmes being unavailable. This paper has implications for the history of PR, contributing to the diversification of the field by suggesting an original approach to the intersection of public relations and social change.
Originality/value
This paper surfaces overlooked primary sources and is the first account of how birth control appeared as a topic on early British broadcast media.

On the whole, interest shown in career roles increased in most areas across the represented age groups and from participants of both genders.
