Abstract
Recent changes in UK immigration policy have led to increasing deportation of foreign national offenders and more than 5000 were deported in 2008. This rise follows a review in 2006 which found that some foreign national offenders were being lost due to disposal or transfer through mental health services. As a result, a department was set up within the United Kingdom Border Agency (UKBA) to identify and review those detained under Restrictions Orders who were due to be conditionally released from hospital, so that they could be considered for deportation. As a result of these changes, forensic clinicians are more frequently presented with detained patients who have immigration problems that can lead to problems in the care pathway through secure services. A local audit was carried out in an inner‐London medium secure service to quantify the number of detained patients who are born abroad, and to review their care pathways to determine whether they are affected by immigration difficulties. It is hoped that the discussion of the issues identified by this audit can support service improvement and provide better care for patients in medium secure services.

Abstract
Alcohol‐related brain damage (ARBD) is an increasing challenge for service providers working with older people and adults. It has a complex aetiology and does not progress in the same way as other causes of dementia. The devastating effects of ARBD undermine a person's ability to lead an independent life, yet it is thought that with the right interventions, a degree of recovery can be seen in 75% of sufferers. People with ARBD do not neatly fit into an existing category of care; they ‘fall through the net’ at multiple points in the care pathway. Using a patient synopsis drawn from clinical practice the author illustrates the impact of an advanced practice role in relation to the nursing care of patients with ARBD, as well as making suggestions for the provision of education and support for mainstream services.

Abstract
Purpose
This paper analyzes scholarly literature and the development of a nonstate food strategy in Canada, the Conference Board of Canada's Canadian Food Strategy, to explore the role of the administrative state in food policymaking.
Design/methodology/approach
This research is based on an exploratory case study drawing data from 38 semistructured interviews, including elite interviews. It also draws on policy documents from the nonstate food strategy.
Findings
This paper shows that various nonstate actors, including large food industry players, identify a role for the state in food policy in two ways: as a “conductor,” playing a managing role in the food policy process, and as a “commander,” taking control of policy development and involving nonstate actors when necessary. The complex and wicked aspects of food policy require the administrative state's involvement in food policymaking, while tamer aspects of food policy may be less state-centric.
Originality/value
This paper fills gaps in studies exploring food policymaking processes as well as the administrative state's role in food policymaking in a governance era. It contributes to a better understanding of the state's role in complex and wicked policy domains.

Abstract
Purpose
As the US health system moves to value-based care and aligns payment with quality, the role of the primary care provider (PCP) is becoming ever more important. The purpose of this paper is to outline a successful population health and care management strategy depending on accountable teams to standard workflow and agreed upon process and outcome measures in order to achieve the triple aim of improved health, patient experience, and value.
Design/methodology/approach
Two major areas of focus for primary care are ensuring that all patients receive appropriate evidence-based screening and prevention services and coordinating the care of patients with chronic conditions. The former initiative will promote the general health and well-being of patients, while the latter is a key strategy for achieving better outcomes and reducing costs for patients with chronic conditions.
Findings
To achieve these goals while managing a busy practice requires that the authors leverage the PCP by engaging clinical and non-clinical team members in the care of their patient population. It is essential that each team member’s role be clearly defined and ensures they are working at the top of their scope.
Originality/value
This initiative was successful because of the compelling objectives, the buy-in generated by using Lean methodology and engaging the team in the design process, use of multiple feedback mechanisms including stories, dashboards, and patient feedback, and the positive impact on providers, staff, and patients.

In my article on the New World of Work I alluded to three stages of business development and to their implications for work and employment. Via the ‘creative re‐integration of business’, I also conjured up a new world of business, where ‘intrapreneurs’ and ‘enablers’ came together with managers and entrepreneurs, and with consultants and craftsmen. In this follow up piece I want to focus on ‘Creative Re‐integration’, as our next step in business development. As a result, I shall be: • making the case for ‘Business Development’ as a new and vital, though hitherto neglected framework, for thinking about organisations • drawing together the economic, social and technological threads that are converging upon us, resulting in a genuinely new world of business • citing examples of innovative moves, within major corporations, towards creative re‐integration.

The relationship between strategic positioning and the appropriate mechanism for its implementation is important to most top managers. The determination of an appropriate organisational relationship will largely reflect the complexity of the trading environment. Intrapreneurship within traditional organisational structural formats is one mechanism to combat turbulent trading conditions. A “menu” of organisational relationships is presented which are used within a looser model of organisational relationships. Changes of this nature imply a re‐examination of managerial “grooming”. Educators must design school curricula around criteria of increasing self‐reliance and risk taking. Management developers should encourage the same characteristics. A change in political and social attitudes is also indicated.

It is now fashionable to suggest that the Celtic regions of the United Kingdom are the internal colonies of the central English state and that they have been, particularly since the rapid industrialization of the nineteenth century, subject to a penetrating anglicization of their culture and institutions. In terms of the internal colonialism thesis, it can be argued that the cultural nationalism of Scotland which was developed in the nineteenth century was an attempt to maintain the distinctiveness of civil society in Scotland in the context of massive regional economic imbalance. The Scottish intelligentsia, dominated by Edinburgh lawyers and Presbyterian ministers, can thus be compared with the intelligentsia of Third World societies undergoing a process of de‐colonization where separate cultural identities have to be preserved or, if necessary, constructed.

Abstract
In this study we aim to examine a Durkheimian solution to the problem of social cooperation. Drawing on relevant literature on rituals and social solidarity, we make a case that both synchronous and complementary ritualistic acts can promote social cooperation by strengthening solidarity.
We used a lab experiment in which participants performed either synchronous, complementary, or uncoordinated group drumming. After the drumming, they self-reported their positive affect, feeling of being in the same group and trust. Then they played a five-round public goods game in which their levels of cooperation were observed.
We found both synchrony and complementarity help sustain group cooperation. Participants who drummed synchronously or complementarily contributed more to the public good than those in the baseline condition, especially in later rounds of the game. Individuals in the synchronous and complementary conditions also showed stronger feelings of being in the same group. Mediation analysis confirmed that the effects of ritual performance on cooperation are partially mediated by feelings of same-groupness.
Results of our study imply that ritual performance based on either members’ similarities or complementary differences can promote group solidarity and cooperation.
The study supports the classic Durkheimian solution to the problem of social cooperation. Consistent with recent research, we find the causal effect of synchrony on cooperation. Moreover, our new test of the effect of complementarity shows that being different but mutually supportive can effectively enhance solidarity and cooperation as well.
