Abstract
Purpose
Post-traumatic zygomatic osteotomy, fracture reduction, and orbital floor reconstruction pose many challenges for achieving a predictable, accurate, safe, and aesthetically pleasing result. This paper aims to describe the successful application of computer-aided design (CAD) and additive manufacturing (AM) to every stage of the process – from planning to surgery.
Design/methodology/approach
A multi-disciplinary team was used – comprising surgeons, prosthetists, technicians, and designers. The patient’s computed tomography scan data were segmented for bone and exported to a CAD software package. Medical models were fabricated using AM; for diagnosis, patient communication, and device verification. The surgical approach was modelled in the virtual environment and a custom surgical cutting guide, custom bone-repositioning guide, custom zygomatic implant, and custom orbital floor implant were each designed, prototyped, iterated, and validated using polymer AM prior to final fabrication using metal AM.
Findings
Post-operative clinical outcomes were as planned. The patient’s facial symmetry was improved, and their inability to fully close their eyelid was corrected. The length of the operation was reduced relative to the surgical team’s previous experiences. Post-operative scan analysis indicated a maximum deviation from the planned location for the largest piece of mobilised bone of 3.65 mm. As a result, the orbital floor implant which was fixed to this bone demonstrated a maximum deviation of 4.44 mm from the plan.
Originality/value
This represents the first application of CAD and AM to every stage of the process for this procedure – from diagnosis, to planning, and to surgery.

WE are as yet without the means of verifying the forecast in our last issue that there would be a cutting of library estimates this year. There has been some; one library authority known to us having ordered a five per cent. cut. The fact still is that such retrenchments can only be made at the expense of a very few items of our estimates, and of these the book fund is the severest sufferer. This is a fact that cannot always be brought home to finance committees who only think as a rule in gross figures, and do not know that the library estimate consists mainly of fixed charges. The librarian who forms an estimate in anticipation of cuts does not aft quite honestly, but in the way of the world at present he may, like the Unjust Steward, be acting wisely. But it is pitiful to think that this should even be a matter for discussion.

The Corporation of the City of London are about to appoint a Public Analyst, and by advertisement have invited applications for the post. It is obviously desirable that the person appointed to this office should not only possess the usual professional qualifications, but that he should be a scientific man of high standing and of good repute, whose name would afford a guarantee of thoroughness and reliability in regard to the work entrusted to him, and whose opinion would carry weight and command respect. Far from being of a nature to attract a man of this stamp, the terms and conditions attaching to the office as set forth in the advertisement above referred to are such that no self‐respecting member of the analytical profession, and most certainly no leading member of it, could possibly accept them. It is simply pitiable that the Corporation of the City of London should offer terms, and make conditions in connection with them, which no scientific analyst could agree to without disgracing himself and degrading his profession. The offer of such terms, in fact, amounts to a gross insult to the whole body of members of that profession, and is excusable only—if excusable at all—on the score of utter ignorance as to the character of the work required to be done, and as to the nature of the qualifications and attainments of the scientific experts who are called upon to do it. In the analytical profession, as in every other profession, there are men who, under the pressure of necessity, are compelled to accept almost any remuneration that they can get, and several of these poorer, and therefore weaker, brethren will, of course, become candidates for the City appointment.

IT is too early to examine what the change of Government may portend for libraries sustained attract malign attention from any party. We are aware enough, however, that a time of financial stringency lies ahead for every public activity. In book production, the restrictions on imports may worsen a position which is bad enough as it is. There may not be a sinister intention in the gesture of cutting the salaries of Cabinet Ministers by a sum which for several of them represents about £25 or about a half crown a week on such salaries as librarians earn. We hope there is not. Although all good Britons will make necessary sacrifices; but they want to be sure that they are necessary and not, as usually is the case, merely attacks on public servants. We are told that there will be no Geddes axe this time, but experience shows that the politician can always find a way of reversing a statement in what he imagines to be the public interest. Purpose
The misuse of pharmaceutical opioid analgesics is identified as a global public health concern. Codeine represents an interesting quandary in terms of its regulated status, with individuals varying in their metabolism of codeine, estimation of safe dosages, risk of adverse health consequences and abuse potential. Efforts to quantify and address hidden non-compliant medical codeine use, overuse and intentional misuse is compromised by availability to the public in prescribed and over the counter forms. The paper aims to discuss these issues.
Design/methodology/approach
A review of literature on codeine use, misuse and dependence, and associated innovative medical and pharmacy interventions is presented, and was conducted as part of a larger scoping review on codeine.
Findings
The review highlights the complexities associated with monitoring public health awareness of codeine's abuse potential, and customer/patients trends in non-compliant codeine use for therapeutic and recreational purposes. Aberrant codeine behaviours centre on visiting multiple doctors for prescriptions, repeated lost or stolen prescriptions, forging prescriptions and use of multiple pharmacies. Innovations to monitor misuse of codeine include national prescription databases and recent developments in real-time monitoring of dispensing activity.
Practical implications
Further development of real-time monitoring processes with process evaluation is advised.
Originality/value
This viewpoint is intended to demonstrate how efforts to quantify and address codeine use are compromised by its availability. It intends to encourage further policy and practitioner dialogue on how to monitor, support and intervene with consumers misusing codeine.

Abstract
Purpose
The purpose of this paper is to review cases of artificial reefs built through additive manufacturing (AM) technologies and analyse their ecological goals, fabrication process, materials, structural design features and implementation location to determine predominant parameters, environmental impacts, advantages, and limitations.
Design/methodology/approach
The review analysed 16 cases of artificial reefs from both temperate and tropical regions. These were categorised based on the AM process used, the mortar material used (crucial for biological applications), the structural design features and the location of implementation. These parameters are assessed to determine how effectively the designs meet the stipulated ecological goals, how AM technologies demonstrate their potential in comparison to conventional methods and the preference locations of these implementations.
Findings
The overview revealed that the dominant artificial reef implementation occurs in the Mediterranean and Atlantic Seas, both accounting for 24%. The remaining cases were in the Australian Sea (20%), the South Asia Sea (12%), the Persian Gulf and the Pacific Ocean, both with 8%, and the Indian Sea with 4% of all the cases studied. It was concluded that fused filament fabrication, binder jetting and material extrusion represent the main AM processes used to build artificial reefs. Cementitious materials, ceramics, polymers and geopolymer formulations were used, incorporating aggregates from mineral residues, biological wastes and pozzolan materials, to reduce environmental impacts, promote the circular economy and be more beneficial for marine ecosystems. The evaluation ranking assessed how well their design and materials align with their ecological goals, demonstrating that five cases were ranked with high effectiveness, ten projects with moderate effectiveness and one case with low effectiveness.
Originality/value
AM represents an innovative method for marine restoration and management. It offers a rapid prototyping technique for design validation and enables the creation of highly complex shapes for habitat diversification while incorporating a diverse range of materials to benefit environmental and marine species’ habitats.
