Abstract
The aim of this study is to quantify prescribing errors relating to pre‐admission medication in patients admitted to hospital. It also assesses the impact of a hospital pharmacist in identifying and correcting these errors. Standard prescription monitoring by the pharmacist took place on admission in phase 1 (526 patients). This was compared with an extension of the pharmacist’s role in phase 2 (506 patients) by taking a detailed medication history, including assessment of patients’ own drugs (PODs). A significant increase (p < 0.001) in errors detected and corrected was identified. In phase 2, 45.1 per cent of prescription charts contained one or more errors. Of these errors, 74.8 per cent could not be identified from checking the drug chart alone. As this is the principal method by which many hospital pharmacists check patients’ medication charts this is an area of concern. More than 50 per cent of errors were considered likely to cause destabilisation of a chronic medical condition, or serious adverse events. It has been shown that an effective way to prevent these errors and ensure the continuity of medication between primary and secondary care is for pharmacists to take a detailed drug history on admission.

Abstract
Purpose
The purpose of this paper is to describe processes of learning from personal experiences of mental distress when mental health service users participate in occupational therapy education with tutors and students who have also had experiences of mental distress.
Design/methodology/approach
A post-structural theoretical perspective was applied to stories which emerged from the research process. Semi-structured group and individual interviews were used with three service users, three students and three tutors (including the author) who had all had, at some time in their lives, experiences of mental distress.
Findings
Stories based on previously hidden personal experiences of mental distress began to shift dominant understandings. Further, as educators, service users challenged whose authority it is to speak about mental distress and permitted different narrative positions for students and tutors. However, technologies of power and technologies of self of powerful discourses in professional education continued to disqualify and exclude personal knowledges. Learning from stories requires a critical approach to storytelling to expose how hidden power relations maintain some knowledges as dominant. Further, learning requires narrative work, which was often hidden and unaccounted for, to navigate complex and contradictory positions in learning.
Social implications
Although storytelling based on personal experience can help develop a skilled and healthy mental health workforce, its impact will be limited without changes in classrooms, courses and higher education which support learning at the margins of personal/professional and personal/political learning.
Originality/value
Learning from stories of mental distress requires conditions which take account of the hidden practices which operate in mental health professional education.

Abstract
Purpose
Currently, many studies have shown an increasing interest in owner-dynamic capabilities (ODCs). Existing studies mainly focus on the dynamic capability basis and capability development within the owner organization, whereas they rarely analyze the capability mobilization within the network of participants in megaprojects. Therefore, this study aims to explain the interaction and evolution of the mobilization strategies of ODCs and the cooperative strategies of other participants.
Design/methodology/approach
This study develops a tripartite evolutionary game model to analyze the evolutionarily stable strategy of the owner, the reciprocal participants and the general participants. Results are numerically simulated with a validation case. The asymptotic stability of multiple group strategies is discussed under the replicator dynamic system.
Findings
This study suggests that resource complementarity significantly reduces the difficulty of mobilization. Moreover, these strategies are only effective with sufficient ODCs. The results indicate that reciprocal participants are more sensitive to the change in resource complementarity.
Originality/value
This study provides strategic guidance for mobilizing ODCs in megaprojects to better embrace uncertainty and stress, contributing to the dynamic capability literature with an evolutionary game approach. And new insight for the study of reciprocity preference in megaprojects is also provided.

Purpose
Women and marketing have had a complicated relationship for a considerable time. They have often been involved with marketing‐type practices for longer than we have appreciated to date. Against considerable odds, some have carved out careers in academia and practice that have to be admired. The purpose of this paper is to explore the work of two pioneer contributors to marketing.
Design/methodology/approach
This paper engages in a close reading of the work of two female contributors. Their writing is placed in historical context which helps reveal the obstacles they had to overcome to succeed.
Findings
Female teachers, lecturers and practitioners had an important role to play in theorising consumer practice and helping people to successfully negotiate a complex marketplace replete with new challenges, difficulties and sometimes mendacious marketers seeking to profit from the limited knowledge consumers possessed.
Originality/value
This paper explores the writings of a practitioner and scholar respectively whose work has merited only limited attention previously. More than this, it links the arguments that are made to the papers that appear in the rest of the special issue.

Purpose
The purpose of this article is to explore some of the biases that affect the classification of Welsh art materials and to examine how they are being perpetuated both in library classification systems and beyond.
Design/methodology/approach
A discourse analysis, in the loosest sense, was used to explore the research topic. Using a hermeneutic and interpretative approach facilitated an examination of some of the tacit assumptions and conceptions that shape the way in which Welsh art is spoken about, thought about, and generally represented.
Findings
The paper explores biases in the classification of Welsh art in relation to the analytical categories of dispersion, dilettantism, and depreciation. Evidence is drawn from three examples of discursive practice: the application of Library of Congress subject headings in the library in Howard Gardens; the Salisbury Collection classification scheme at Cardiff University; and the descriptive text taken from the web site of the National Museum, Cardiff. The paper concludes with a discussion of the nature of classification, and the role of the information professional as active player in the practice of representation in and through various methods of classification.
Originality/value
The paper contributes to the literature of classification bias. The focus on the specific rather than the more general biases both adds to Olsen and Schlegl's work and reflects a sensitivity to the subject matter itself. The paper also contributes to the literature at a methodological level in its use of a hermeneutic and interpretative analytical framework to explore representation in classification.

Abstract
Purpose
In the age of Industry 4.0, digital advancement is reshaping manufacturing models towards product–service systems (PSS). The drivers, readiness and challenges to move to a PSS model are not well understood, and the exploitation of the digital era presents the gap of this research.
Design/methodology/approach
The research was conducted using semi-structured interviews in six manufacturers. Two forum debates were also conducted to supplement and validate the findings.
Findings
Social and economic motivations rather than environmental considerations were driving the change to PSS. Digital technologies could be an important driver if manufacturers reached a certain PSS maturity level. A high level of technical readiness was offset by a low level of social investments and the strategic development of human resources. Value co-creation was a main challenge though manufacturers had the advantage of digital connectivity, which indicated new human requirements; the greater the enabling power of digital technologies, the greater the need for advanced human skills.
Practical implications
Human resource management has underpinned lean models; yet, the role of employees within PSS is underdeveloped despite the impact of staff in exploiting digitalisation and value co-creation. A “learning organisation” and socio-technical fit are required for the “diffusion of innovation” of PSS.
Originality/value
This research attempted to explore drivers, readiness and challenges for PSS from a socio-technical systems (STS) perspective. Three levels of PSS maturity with STS features were derived from the research, providing guidance for manufacturers.

We are the midst of accelerated change in the academic workforce. Academic roles, being a resilient mixture of research and education, are differentiating. An education-focused academic role, rather than one solely focused on disciplinary research, is gaining credibility and value. With the rise of the education-focused academic roles, questions are being raised about whether this new form of education-focused academic will continue to be overly represented by women in a new form of “women's work.” In the next decade, as academic roles continue to differentiate, care needs to be taken not to repeat the practice of the last 100 years which has seen gender bias continue. We will present four profiles of education-focused women academics, the snakes and ladders in their careers, and the strategies needed to ensure that women progress with equal recognition in these complex but exciting times.

Innovation is a growing topic. The primary sector is also a subject that has great importance in the global economy. However, limited research has been conducted linking these two subjects. This paper aims to start bringing them together.
Design/methodology/approach
The method selected is a systematic literature review presenting a thorough analysis of the existing discussions and a summary of the findings to identify future research opportunities.
Findings
Innovation has played and will continue to play an essential role in the success of fisheries. The fishing industry is in crisis. A decline in wild fish stocks, regulations and quota management are some of the factors that drive the need to innovate. The discussions of innovation in this industry are non-technological, which is an emerging field with opportunities for further research. There is a strong focus on fisheries sustainability. The main theoretical frameworks discussed in the literature are common property theory, cooperation and competition theory and diffusion of innovation. Emerging environmental sustainability frameworks are also becoming an essential element in the primary sector.
Research limitations/implications
This paper focuses on peer-reviewed publications based on the results of the searches from selected keywords, recognizing that the literature evaluated may not cover the full spectrum of themes related to this subject.
Originality/value
Innovation is usually linked to high tech and fast-moving industries. This paper tries to broaden this paradigm and evaluate it from a different perspective, with a focus on the primary sector.
