Abstract

Purpose

The purpose of this paper is to investigate perceptions towards a career in psychiatry among medical students and psychiatrists and identify how recruitment into the specialty may be improved.

Design/methodology/approach

This study locally compares medical student and psychiatric doctor responses to a structured online survey and structured interviews with key managerial figures in the Humber NHS Foundation Trust.

Findings

Comparison across two main areas (pre-decision exposure to psychiatry and reasons for considering a psychiatric career) found that both students and doctors were influenced to make a choice about a career in psychiatry during medical school. Medical students found compatibility with family life to be more important when considering psychiatry, whereas doctors cited content-based reasons as significant pull factors. Stigma and fear of being harmed deterred some students from choosing a career in psychiatry. Structured interview responses reiterated the importance of pre-medical school and undergraduate mentorship in bolstering future recruitment to psychiatry.

Practical implications

Medical students perceive certain career issues differently to their postgraduate counterparts. Widening the content-based appeal of psychiatry and optimising the medical school experience of the specialty via varied and high-quality placements may be a key step towards tackling the national shortfall in qualified psychiatrists.

Originality/value

This is the first published study comparing medical student and psychiatric doctor perceptions of a career in psychiatry.

Abstract

Purpose

This study documents the role of relational trust in an afterschool organization and its influences on young people’s experiences.

Design/methodology/approach

Through a 10-month ethnographic study of one afterschool program that teaches teens how to make documentaries, I demonstrate that the confluence of blurred organizational goals; weak relational trust among staff; and funding pressures may have the unintended consequence of exploiting students for their work products and life stories.

Findings

The study finds that, while not all organizations function with student work at its center, many afterschool organizations are under increasing pressures to document student gains through tangible measures.

Practical implications

Implications from these findings reveal the need for developing strong relationships among staff members as well as establishing transparency in funding afterschool programs from within the organization and from foundations in order to provide quality programming for young people.

Originality/value

This study informs organizational theory, specifically in terms of measures of variation in relational trust within an organization and its influence on young people. This chapter includes student accounts of experiences with staff to enhance the significance of relational trust.

Abstract

Purpose

Community Makers (https://communitymakers.co) is an active UK wide network that evolved rapidly in response to COVID-19 and the negative impact of social isolation and distancing on people and families affected by dementia. The network is led by the Alzheimer’s Society, UK Dementia Research Institute Care Research and Technology Centre and The Association for Dementia Studies, at the University of Worcester. This study aims to investigate what works for whom and why and in what circumstances as a basis for the development of an online toolkit for people supporting people affected by dementia to connect and reconnect during the pandemic and beyond.

Design/methodology/approach

The development of the toolkit was informed by an iterative approach of engagement with people affected by dementia, learning from a network of community organisations and a rapid realist literature review.

Findings

Four common factors that combine to make a successful support intervention, regardless of its type, size or location were identified: relationships, purpose, technology and community. The application of the factors, adopted as principles, is illustrated by applying them to three real-world examples.

Social implications

The main output was the online tool kit, which is a resource of creative ideas to inspire groups with different approaches to digital involvement and help reduce social isolation.

Originality/value

This report offers new insight, based on identification of four principles, into how people affected by dementia can be supported online and offline during a pandemic and beyond.

Abstract

Purpose

Change fatigue refers to the state when excessive change has led workers to feel exhausted and unable to further adapt. While the concept of change fatigue has been discussed, research exploring predictors, mediators and consequences of change fatigue is limited. The purpose of this study was to empirically demonstrate that organizational change frequency predicts change fatigue, and that change fatigue predicts important outcomes (e.g. reduced performance) via mediators such as reduced commitment and satisfaction.

Design/methodology/approach

In two cross-sectional studies, the authors explored predictors, mediators and outcomes of change fatigue.

Findings

In study one, participants from organizations experiencing more change reported greater change fatigue, and change fatigue predicted increased strain, burnout, intention to turnover and decreased engagement. In study two, change fatigue had significant indirect effects on teamwork, turnover intention and performance via reduced job satisfaction and organizational commitment.

Research limitations/implications

Both studies were cross-sectional; future studies should explore the predictors and consequences of change fatigue longitudinally.

Practical implications

Change managers need to be aware that frequent organizational changes predict change fatigue, which reduces both job satisfaction and organizational commitment and leads to worse performance.

Originality/value

This is the first study demonstrating that change frequency predicts change fatigue, and that fatigue impacts performance outcomes via reduced satisfaction and commitment.

Abstract

Purpose

The purpose of this paper is to evaluate a collaborative effort between a health care organization and academic institution to strengthen organizational health literacy.

Design/methodology/approach

The intervention took place at a rural, federally qualified health clinic in Missouri between May 2009 and April 2011. Qualitative interviews of key informants were conducted before (n=35) and after (n=23) the intervention to examine program implementation and success in effecting organizational change.

Findings

Intervention activities helped establish a comprehensive understanding of health literacy. The project achieved moderate, fundamental and sustainable organizational change. The program successfully integrated health literacy practices into clinic systems and garnered leadership and organizational commitment, helped the workforce improve interpersonal communication and embedded practices making health education materials more accessible.

Originality/value

The study points to programmatic, conceptual and methodological challenges that must be addressed for organizations to improve health literacy practices, and suggests change management strategies to advance organizational health literacy.

Abstract

Education, and especially the life of a teacher, is constantly evolving. Teachers' roles and responsibilities have become more complicated and multifaceted over time. Therefore, they have to invest a great deal of time, energy, and effort to navigate the demands and complexities of their role. In the last 20 years, education has changed in response to increased workload, the rise of high-stakes testing and accountability, social-emotional and behavioral challenges among students, school safety concerns, equity concerns, and now the coronavirus pandemic. These factors have been linked to high stress and teacher burnout, which leads to more teachers leaving their jobs (Arvidsson et al., 2019). Nonetheless, those who endure the flames of burnout do so because they are resilient in the face of adversity and persistent in their commitment to the call of teaching and learning. This chapter examines the effects of stress and burnout among three alternatively certified teachers and the behaviors and strategies these educational first responders employ to build resilience and persistence in their service to the students who shape our future.

Abstract

The focus on local-level policy initiatives in US anti-fracking movements presents unique opportunities to explore interactions between professional advocacy organizations with regional/national constituencies and grassroots organizations with constituencies who will directly experience changes in local landscapes resulting from unconventional oil and gas development (UOGD). However, research on anti-fracking movements in the US has considered dynamics of interorganizational cooperation only peripherally. This chapter examines factors that motivate coalition building, sources of coalition fragmentation, and the progressive polarization of grassroots anti-fracking and countermovement activists using qualitative research on an anti-fracking movement in Illinois. While grassroots groups may experience some strategic advantages by collaborating with extra-local, professionalized advocacy organizations, these relationships involve navigating considerable inequalities. In the case presented here, I find that coalition building was important for putting UOGD on the policy agenda. However, when anti-fracking activists began experiencing success, institutionalization rapidly produced fragmentation in the coalition, and a countermovement of UOGD supporters was formed. I highlight how ordinary movement dynamics are particularly susceptible to polarization in the context of local land use disputes that “scale-up” to involve broader movement constituencies as perceptions of distributive injustice collide with perceptions of procedural injustice.

Abstract

The aim of this article is an analysis of the links between race and psychotic illness, psychiatric diagnosis and treatment, as well as psychiatric, police and prison violence against people with mental health problems. The analysis focuses on Black men who are more frequently diagnosed with schizophrenia and other psychotic disorders and who face more brutal treatment than other people with such diagnoses. We have adopted a multidisciplinary approach which draws insights from psychiatry, psychology, and sociology and challenges the biologistic interpretation of “mental illness.” We take into account the United States and Britain – two countries with large Black minorities and an established tradition of research on these groups. Among the crucial findings of this study are the facts that racial bias and stereotypes heavily influence the way Black men with a diagnosis of psychotic illness are treated by the psychiatric system, police and prison staff, and that the dominant approach to psychosis masks the connections between racism and mental health.

Abstract

Purpose

The study examines health care inequities in viral load testing among hepatitis C (HCV) antibody-positive patients. The analysis predicts whether individual and census tract sociodemographic characteristics impact the likelihood of viral load testing.

Methodology/Approach

This a study of 26,218 HCV antibody-positive patients in Orange County, California, from 2010 to 2020. The case data were matched with the 2017 American Community Survey to help understand the role of neighborhood socioeconomic characteristics in testing for viral load. Multivariable logistic regression was used to predict the probability of ever testing for HCV viral load.

Findings

Thirty-six percent of antibody-positive persons were never viral load tested. The results show inequalities in viral load testing by sociodemographic factors. The following groups were less likely to ever test for viral load than their counterparts: (1) individuals under 65 years old, (2) females, (3) residents of census tracts with lower levels of health insurance enrollment, (4) residents of census tracts with lower levels of government health insurance, and (5) residents of census tracts with a higher proportion of non-white residents.

Research Limitations/Implications

This is a secondary database from public health department reports. Using census tract data raises the issue of the ecological fallacy. Detailed medical records were not available. The results of this study emphasize the social inequality in viral load testing for HCV. These groups are less likely to be treated and cured, and may spread the disease to others.

Originality/Value

This chapter is unique as it combines routinely collected public health department data with census tract level data to examine social inequities associated with lower rates of HCV viral load testing.

Abstract

Purpose

In the context of social media (SM) use, self-disclosure (SD) behaviour meets users' social and emotional needs, but it is also accompanied by risks that can harm users. This paper aims to identify the factors that influence users' SD behaviour on SM in Indonesia, using a comparative analysis based on age groups.

Design/methodology/approach

A survey was conducted on 2,210 respondents who were active SM users in Indonesia. Data were processed and analysed using covariance-based structural equation modelling with AMOS 24.0 software.

Findings

Results indicate that, in the overall age group data, factors such as use of information (UI), trust, privacy control (PC), interactivity, perceived benefits (PB) and perceived risks (PR) influence users' SD behaviour. This research also found differences in the characteristics of SD behaviour between age groups.

Originality/value

Findings from this study can help SM service providers to evaluate the credibility and reliability of their platforms to encourage user retention.
