Abstract
Purpose
One of the major challenges in practicing family medicine residents (FMRs) is the choice of an appropriate pattern in training health centers (THCs) to provide high-quality health services to patients. This study aimed to design an efficient residency training model in Iranian healthcare centers.
Design/methodology/approach
A four-phase qualitative study with the nominal group technique was carried out in 2018–2019. The required standards of THCs and superior educational practices were initially extracted from the review literature. After collecting and presenting the key findings to the experts' panel, the required training standards and the optimal strategic model to educate FMRs in THCs were prioritized by a nine-point rating system for the well-structured terms of indigenous resources.
Findings
Twenty-two educational standards for FMRs in THCs were finalized after reviewing the literature and taking an opinion poll of experts. The highest assessment scores belonged to sufficient numbers of referrals for common health issues and the existence of satisfactory educational and recreational facilities in THCs. The problem-based (PBL), task-based (TBL) and context-based (CBL) learning models were better strategies than guided-discovery learning (GDL) and small group teaching (SGT) ones to train FMRs in THCs. Also, PBL and SGT models were rated as the best and worst advanced medical education methods to empower FMRs.
Originality/value
Due to different resources available in THCs and training centers, different factors and infrastructures should be considered to meet the required standards in building the integrative training model for FMRs in THCs.

Abstract
Purpose
Hospital evaluations create competition between healthcare providers. In this study, a multi criteria decision-making (MCDM) method is used to evaluate criteria that affect hospital service quality. The paper aims to discuss these issues.
Design/methodology/approach
Criteria affecting hospital service quality are identified. Four Iranian public hospitals are evaluated using these criteria. Four hybrid methods, including modified digital logic–technique for order of preference by similarity to an ideal solution, analytical hierarchy process–technique for order of preference by similarity to an ideal solution, analytical hierarchy process–elimination and choice expressing reality and modified digital logic–elimination and choice expressing reality are used to evaluate hospital service quality. Results are aggregated using the Copeland method and final ranks are determined.
Findings
The four main criteria for evaluating hospital service quality are: environment; responsiveness; equipment and facilities; and professional capability. Results suggest that professional capability is the most important criterion. The Copeland method, used to integrate four MCDM hybrid methods, provides the final hospital ranks.
Practical implications
The criteria the authors identified and their weight help hospital managers to achieve comprehensive organizational growth and more efficient resource usage. Moreover, the decision matrix helps managers to identify their strengths and weaknesses.
Originality/value
New and comprehensive criteria are proposed for hospital quality assessments. Moreover, a new hybrid MCDM approach is used to achieve final hospital rankings.

Abstract
Purpose
Some studies on anthocyanins have revealed their antioxidant activity and beneficial effects for diabetes control and reducing the risk of coronary heart diseases. It has been found that sour cherries contain high levels of anthocyanins that possess insulin‐releasing stimulatory properties on pancreatic β‐cells in vitro. The purpose of this paper is to investigate whether concentrated sour cherry juice (CSCJ) beneficially alters serum glucose and some cardiovascular risk factors in diabetes type 2 subjects.
Design/methodology/approach
In this quasi‐experimental study, 19 diabetic women with FBS ≥ 110 mg/dl were recruited from patients referred to the Diabetes Clinic of Shariati Hospital. Subjects were asked to consume 40 g of CSCJ daily for 6 weeks. Before the onset of the study (week 0) and after 6 weeks, weight and blood pressure measurements were done and fasting blood samples were drawn. FBS, hemoglobin A1c (HbA1c) and blood lipid profiles were measured. In addition, a 24‐hour food record was taken from all of the individuals in both stages. The Wilcoxon signed test was used for statistical analysis.
Findings
After six weeks' consumption of CSCJ, significant reductions in body weight (p < 0.01), blood pressure and HbA1c (p < 0.05) was seen. Total cholesterol and LDL‐C decreased significantly in a sub‐group of patients (n = 12) with LDL‐C ≥ 100 mg/dl as well.
Originality/value
Based on the results of this study, consuming 40 g/day of CSCJ decreases body weight, blood pressure and HbA1c in diabetes type 2 women after 6 weeks and improves blood lipids in diabetic patients with hyperlipidemia.

Abstract
Purpose
Health tourism is a relatively new branch of international tourism that has developed more rapidly than other tourism sectors in recent years. This study aimed to investigate the effect of government supports on health tourism development by considering the mediating role of internal and external infrastructures.
Design/methodology/approach
The study population consisted of all experts of two hospitals in Iran, which are frequently visited by foreign tourists (N = 151). A questionnaire, developed by combining standard and researcher-made questionnaires, was used to collect the data. The confirmatory factor analysis (CFA) model was developed in SmartPLS 3 to test the hypotheses.
Findings
The main hypothesis test results indicated that government supports positively affect the development of health tourism. Internal and external infrastructures were also found to mediate the relationship between government supports and health tourism development. In addition, the sub-hypothesis test results showed that internal and external infrastructures are positively affected by government supports, which puts forth the development of health tourism. As the results explained, the most important aspects of internal infrastructures affected by government support were health service quality, cost of health services and applying advanced medical technologies, respectively. Also, different aspects of external infrastructures affected by government supports are as follows: economic, infrastructures and cultural factors.
Originality/value
This study is the first of its kind to examine the impact of both medical and non-medical factors on health tourism and signifies the crucial role of governments in the development of health tourism.

Abstract
Purpose
The purpose of this study is to identify the situation of spatial inequality in the residential system of Tehran city in terms of housing prices in the year 2021 and to examine its changes over time (1991–2021).
Design/methodology/approach
In terms of purpose, this study is applied research and has used a descriptive-analytical method. The statistical population of this research is the residential units in Tehran city 2021. The average per square meter of a residential unit in the level of city neighborhoods was entered in the geographical information system (GIS) in 2021. Moran’s spatial autocorrelation method, map cluster analysis (hot and cold spots) and Kriging interpolation have been used for spatial analysis of points. Then, the change in spatial inequality in the residential system of Tehran city has been studied and measured based on the price per square meter of a residential unit for 30 years in the 22 districts of Tehran by using statistical clustering based on distance with standard deviation.
Findings
The result of spatial autocorrelation analysis with a score of 0.873872 and a p-value equal to 0.000000 indicates a cluster distribution of housing prices throughout the city. The results of hot spots show that the highest concentration of hot spots (the highest price) is in the northern part of the city, and the highest concentration of cold spots (the lowest price) is in the southern part of Tehran city. Calculating the area and estimating the quantitative values of data-free points by the use of the Kriging interpolation method indicates that 9.95% of Tehran’s area has a price of less than US$800, 17.68% of it has a price of US$800 to US$1,200, 25.40% has the price of US$1,200 to US$1,600, 17.61% has the price of US$1,600 to US$2,000, 9.54% has the price of US$2,000 to US$2,200, 6.69% has the price of US$2,200 to US$2,600, 5.38% has the price of US$2,600 to US$2,800, 4.59% has the price of US$2,800 to US$3,200 and finally, the 3.16% has a price more than US$3,200. The highest price concentration (above US$3,200) is in five neighborhoods (Zafaranieh, Mahmoudieh, Tajrish, Bagh-Ferdows and Hesar Bou-Ali). The findings from the study of changes in housing prices in the period (1991–2021) indicate that the southern part of Tehran has grown slightly compared to the average range, and the western part of Tehran, which includes the 21st and 22nd regions with much more growth than the average price.
Originality/value
There is massive inequality in housing prices in different areas and neighborhoods of Tehran city in 2021. In the period under study, spatial inequality in the residential system of Tehran intensified. The considerable increase in housing prices in the housing market of Tehran has made this sector a commodity, intensifying the inequality between owners and non-owners. This increase in housing price inequality has caused an increase in the informal living for the population of the southern part. This population is experiencing a living situation that contrasts with the urban plans and policies.
