Abstract

Purpose

Nutrition plays a key role in the recovery of pediatric trauma patients. A catabolic state in trauma patients may hinder recovery and inadequate nutrition may increase morbidity, mortality and length of hospital stay. The purpose of this paper is to review the current nutrition support practices for pediatric trauma patients at McMaster Children’s Hospital (MCH), describe patient demographics and identify areas to improve the quality of patient care.

Design/methodology/approach

A retrospective chart review was conducted on pediatric trauma patients (age<18 years) identified through the trauma registry of MCH. Pediatric trauma patients admitted from January 2010 to March 2014 with an Injury Severity Score (ISS)=12 and a hospitalization of =24 hours were included.

Findings

In total, 130 patients were included in this study, 61.1 percent male, median age ten years (range: 0-17 years) and median ISS of 17 (range: 12-50). Blunt trauma accounted for 97.7 percent of patients admitted and 73.3 percent had trauma team activation. In total, 93 patients (71.5 percent) had ICU stays. The median time to feed was 29 hours (interquartile range: 12.5-43 hours) from the time of admission. An increased hospital length of stay was associated with longer time to initiation of nutrition support, a higher ISS and greater number of surgeries (p<0.05).

Originality/value

Local nutritional support practices for pediatric trauma patients correspond with recommended principles of early feeding and preferential enteral nutrition. Harmonization of paper-based and electronic data collection is recommended to ensure that prescribed nutritional support is being delivered and nutritional needs of pediatric trauma patients are being met.

Abstract

Purpose

Dating is a meaningful occupation for many single people. The occupation of dating has transformed considerably in Ireland due to recent changes in Irish culture and the advent of online dating technology. The purpose of this study was to explore the complexities and intricacies of dating in an Irish context.

Design/methodology/approach

A qualitative research approach was used and data were collected using semi-structured interviews with ten heterosexual women (age 24‐34) living in urban areas of Ireland. Data were analysed using inductive thematic analysis (Braun and Clarke, 2006).

Findings

Dating did not fully resonate with their experience, but they did not have an alternative term. The form of dating was influenced by cultural, temporal, physical and virtual contexts. Beliefs about dating, fluctuating emotions and feelings of mortification because of the stigma of online dating created meaning for participants and influenced their use of strategies to improve resilience. While dating was not a preferred occupation for participants because of its arduous nature and fluctuations in emotion, it was seen as essential to fulfil the function of finding a romantic partner or partner in occupation. Connections between participant experiences and occupational science are discussed to address the lexicon of dating and the form, function and meaning of dating.

Originality/value

This study contributes to occupational science knowledge by revealing the occupational understandings of dating as an emerging and dynamic occupation in a rapidly changing culture of Ireland.

Abstract

Purpose – The goal of this chapter was to empirically describe the patterns and dimensions of community interaction of typically underserved families of children with autism and other developmental disabilities within a family quality of life (FQoL) context.

Methodology – We utilized the theoretical framework of FQoL to examine the community interactions of 149 families who voluntarily participated in this study. The Family Quality of Life Survey (FQoLS-2006) was used to collect data from the primary caregivers of the children with the disability, 92% of which were mothers. We conducted an in-depth examination of the six dimensions (importance, opportunities, initiative, attainment, stability, and satisfaction) of community interaction.

Findings – Most families viewed community interaction as very important to their FQoL and the majority (62%) were satisfied with their community interaction, although just over a third reported high attainment, and only 48% reported having adequate opportunities for community interaction. Families of children with autism reported lower attainment of community interaction when compared to families of children with other developmental disabilities (t=2.63, df=147, p=0.01). Some race-related and child-related differences were also observed in the initiative taken to pursue community interaction and discrimination experienced by families.

Limitations – Results must be interpreted with caution, as the participants in this study were all volunteers and the majority were mothers, and therefore may not be representative of all families of children with disabilities. Despite the limitations, findings from this study are a first step in understanding the multidimensional nature of community interaction of low-income, minority families.

Abstract

Demographic shifts and increasing diversity have increased calls for more Black women in higher education teaching and leadership in Canada. This chapter examines how I navigate my practical and theoretical journey in academe through my ontological experiences as a Black female immigrant in large university in Southern Ontario Canada. Drawing on critical race theory (CRT) as a theoretical frame, I explore and theorize my resistance to racial microaggressions through what I describe as “navigational moves.” These “navigational moves” include decolonizing education, spirituality, self-care, and developing a supportive network. These “navigational moves” are grounded in my history and experiences. The chapter explores notions of resistance, empowerment, and sustenance as important factors in challenging racial microaggressions in academe.

Currently, transition-age youth on the autism spectrum face a loss of access to services. They are at risk for a number of difficulties, including reduced meaningful relationships, internalizing and externalizing problems, and problems with adaptive skills. In the United States, young adults are expected to either attend postsecondary education and/or pursue gainful employment. However, neurodivergent individuals face barriers and often have difficulties meeting these societal expectations. The following chapter will highlight ways that employers and entrepreneurs can support neurodivergent individuals, including meaningful accommodations and inclusive practices.
