Abstract

Purpose

Guidelines have been produced in nearly all developed countries to provide cost‐effective hospital care. The study's purpose, therefore, was to determine whether a London hospital's anti‐microbial guidelines conform to this principle.

Design/methodology/approach

The paper's approach was a literature search to determine anti‐microbial therapies for certain diseases and comparing outcomes with what hospital guidelines recommend.

Findings

There are significant discrepancies in the hospital anti‐microbial guidelines and what is recommended in the literature.

Research limitations/implications

Local microbial patterns for these diseases were not studied as they were not formally available and these could have had an impact on guidelines recommendations.

Practical implications

Local guidelines influence day‐to‐day hospital clinical practice and their robustness is important. They need to comply with national and/or international guidelines. Deviations from these guidelines need appropriate comments within the documents to highlight their validity. Such an approach would facilitate medical students and junior doctor training who depend on these guidelines for good clinical practice.

Originality/value

The study provides an important contribution to developing hospital clinical guidelines.

Abstract

Purpose

The aim of this paper was, principally, to look at the coroner's post-mortem report quality regarding adult medical patients admitted to an English hospital; and to compare results with Royal College of Pathologists guidelines.

Design/methodology/approach

Hospital clinical notes of adult medical patients dying in 2011 and who were referred to the coroner's office to determine the cause of death were scrutinised. Their clinical care was also reviewed.

Findings

There needs to be a comprehensive approach to coroner's post-mortems such as routinely taking histological and microbiological specimens. Acute adult medical patient care needs to improve.

Research limitations/implications

Steps should be taken to ensure that comprehensive coroner's post-mortems are performed throughout the UK, including with routine histological and microbiological specimens examination. Additionally, closer collaboration between clinicians and pathologists needs to occur to improve emergency adult medical patient clinical care.

Originality/value

The study highlights inadequacies in coroner's pathology services.

Abstract

Purpose

Bangladesh has an outdated healthcare system that has not changed since its independence in 1971. The aim of this paper is to look into how the Bangladesh healthcare system can be reformed.

Design/methodology/approach

This paper is literature‐based and a personal reflection on one country's health system.

Findings

Widespread dissatisfaction exists in the population and amongst doctors about healthcare delivery.

Practical implications

Major issues causing healthcare user dissatisfaction are discussed and recommendations for reform are suggested.

Originality/value

The paper provides an independent account of one country's healthcare policy and practice.
