Abstract

Purpose

Clozapine is a well-known antipsychotic medication licensed for treatment-resistant schizophrenia, but there is limited research available to suggest its efficacy in the context of personality disorder and intellectual disabilities presenting with high-risk behaviour with or without psychotic symptoms. The purpose of this paper is to raise awareness of the benefits of using clozapine in patients with intellectual disabilities and personality disorder that present with a complex picture of serious risk of harm to both their life and the lives of others.

Design/methodology/approach

The authors present five patients with intellectual disabilities and serious life-threatening challenging behaviour whom were started on clozapine as part of their multidisciplinary treatment plan to manage their presentation. The authors completed baseline assessment of five main symptom domains and then repeated this assessment following treatment with clozapine.

Findings

In all five cases use of clozapine was objectively associated with an improvement in symptomatology, quality of life and a safe transfer to the community.

Originality/value

The findings suggest that judicious use of clozapine could be considered as one of the effective pharmacological strategies in the management of patients with intellectual disabilities and personality disorder who present with serious life-threatening challenging behaviours.

Abstract

Purpose

To increase charitable donations, not-for-profit organisations sometimes include a pre-giving incentive such as a monetary gift alongside a request to donate. Little is known about how monetary pre-giving incentives impact subsequent donation intentions, particularly for young consumers who are an important market for the not-for-profit sector. Using the principles of reciprocity, this paper aims to examine whether the value of monetary pre-giving incentives induces obligation (i.e. a negative psychological state) and/or gratitude (i.e. a positive psychological state) and whether this subsequently impact young consumers’ initial donation amount and future donation intention.

Design/methodology/approach

This research uses a one-factor, three-level between-subjects experimental design (n = 274) to test the effect of different values of pre-giving incentives on gratitude, obligation, initial donation amount and future donation intentions of young consumers.

Findings

Higher value pre-giving incentives increase young consumers’ experience of obligation and subsequent donation intention as well as future donation intention. Unexpectedly, gratitude towards the organisation was not influenced by the value of the pre-giving incentive but did increase obligation.

Originality/value

This research highlights an ethical tension about the use of pre-giving incentives to solicit donations from young consumers: while this strategy is effective, it activates an obligation that can negatively impact young consumers’ wellbeing. As such, it also contributes to extending the limited empirical examination of the ethics of fundraising. Theoretically, this research extends understanding of the distinct but simultaneous mechanisms of gratitude and obligation, which has not received sufficient research attention, generated by the norm of reciprocity within the not-for-profit context.

Abstract

THE Reference Department of Paisley Central Library today occupies the room which was the original Public Library built in 1870 and opened to the public in April 1871. Since that date two extensions to the building have taken place. The first, in 1882, provided a separate room for both Reference and Lending libraries; the second, opened in 1938, provided a new Children's Department. Together with the original cost of the building, these extensions were entirely financed by Sir Peter Coats, James Coats of Auchendrane and Daniel Coats respectively. The people of Paisley indeed owe much to this one family, whose generosity was great. They not only provided the capital required but continued to donate many useful and often extremely valuable works of reference over the many years that followed. In 1975 Paisley Library was incorporated in the new Renfrew District library service.

Abstract

This study aims to use pre‐treatment assessment scores to predict the drop out of 103 incarcerated male violent perpetrators undertaking a long‐term aggression programme, namely the Cognitive Self Change Programme (CSCP), in six English prisons. A hierarchy of best predictors of attrition in this sample is developed. Results found eight out of the 46 assessment variables analysed had a significant association with treatment drop out. Further to this, discriminant function analysis predicted group membership with 80% accuracy, successfully distinguishing perpetrators who dropped out of the programme from those who completed it. The findings support the use of identifying risk factors pre‐treatment to predict drop out and offer a practical way to identify group members who are likely to drop out of the CSCP in addition to identifying markers for programme improvement. The need for further research to increase our understanding of the underlying causal explanations that link specific assessment items to treatment dropout is discussed.

Abstract

Despite the rhetoric emphasising partnership working, there has been a dearth of research related to the educational practices that reify interprofessional partnerships for young children with special educational needs. This doctoral study examined the subtle power shifts in the interactions between early years educators and other professionals against the backdrop of deficit policy discourses and institutional challenges. This research adopted a case study approach and utilised methodological triangulation to unveil educators' phronetic knowledge. The findings point to power differentials and partnership inequities which affect the roles and identities of early years educators. Participants assumed emergent leadership roles that encompassed elements of social pedagogy and pedagogical eclecticism which eschewed medicalised interventions in favour of intuitive pedagogical approaches centred on the child and family.

Abstract

Purpose

The research reported here forms part of a study of children’s participation in children in care reviews and decision making in one local authority in England. The purpose of this paper is to outline the views of 11 social workers and 8 Independent Reviewing Officers (IROs) and explores their perceptions of children’s participation in reviews. The paper considers the barriers to young people participating meaningfully in decision making and how practice could be improved in this vital area so that children’s voices are more clearly heard and when possible acted upon by professionals.

Design/methodology/approach

The data reported here derive from a qualitative cross-sectional study in one English local authority. The entire study involved interviewing children in care, IROs, social workers and senior managers about young people’s participation in their reviews. Findings from the interviews with young people and senior managers have been reported elsewhere (Diaz and Aylward, 2018; Diaz et al., 2018); this paper focusses on the interviews with social workers and IROs. Specifically, the authors were interested in gaining insight into their views about the following research questions: To what degree do children and young people meaningfully participate in reviews? What are the barriers to participation? What can be done to improve children and young people’s participation in reviews?

Findings

During this process seven themes were identified, five of which concerned barriers to effective participation and two which concerned factors that appeared to support effective participation. These are summarised below and explained further in the following sections. Barriers to effective participation: social workers and IROs’ high caseloads and ensuing time pressures; high turnover of social workers and inexperienced staff; lack of understanding and training of professionals in participation; children and young people’s negative experiences of reviews and consequent reticence in taking part; and structure and process of the review not being child-centred. Factors which assist participation: quality of the relationship between the child and professionals; and the child or young person chairing their own review meeting.

Research limitations/implications

Although these findings reflect practice in one local authority, their consistency with other research in this area suggests that they are applicable more widely.

Practical implications

The practice of children chairing their own reviews was pioneered by The Children’s Society in North West England in the 1990s (Welsby, 1996), and has more recently been implemented with some success by IROs in Gloucestershire (see Thomas, 2015, p. 47). A key recommendation from this study would be for research to explore how this practice could be developed and embedded more widely. Previous research has noted the tension between the review being viewed as an administrative process and as a vehicle of participation (Pert et al., 2014). This study highlighted practitioner reservations about young people chairing their own reviews, but it also gave examples of how this had been done successfully and how it could improve children’s participation in decision making. At the very least, it is essential that young people play a role in deciding where the review is going to take place, when it will take place, who is going to be invited and what will be included on the agenda.

Social implications

The paper highlights that in this Local Authority caseloads for social workers were very high and this, combined with a high turnover of staff and an inexperienced workforce, meant that children in care struggled to have a consistent social worker. This often meant that young people were not able to build up a positive working relationship with their social worker, which negatively impacted on their ability to play a meaningful role in decision making.

Originality/value

There have been very few recent studies that have considered professionals’ perspectives of children’s participation in key meetings and decision making, so that this provides a timely and worthwhile contribution to this important area of work.

Purpose

Gangs have become a hot topic in recent years, particularly since 2011 when gang members became the poster child for “the worst bout of civil unrest in a generation”. Given the portrayal of gang members as “super predators,” it is maybe not surprising that much of the media and scholarly attention, to date, has focussed on gang members as perpetrators of violence — paying little attention to their role as victims and the impact this may have on their psychological wellbeing (Bennett et al., 1996). The purpose of this paper is to evaluate and synthesize theory and research relating to the relationship between gang membership and mental health problems such as anxiety, depression and post traumatic stress disorder and considered how treating gang violence as a public health problem, rather than punishing it as a criminal justice problem has superior benefits in terms of rehabilitation and reduced recidivism.

Design/methodology/approach

The scarcity of research on this topic meant that research from other subfields of psychology was be collated in order to build a clearer picture of the psychological consequences belonging to a gang can have.

Findings

It is clear from this review that gang members’ involvement in violence (as victims and perpetrators) is likely to have a negative impact on their behavioral, social and psychological functioning.

Originality/value

The authors suggest future directions should be aimed toward developing and honing a robust program of research capable of producing intelligence-led assessment and intervention.

Abstract

Purpose

The purpose of this paper is to investigate the impact of psychological case formulation meetings for staff in a secure forensic learning disability and autism service.

Design/methodology/approach

In total, 89 of the attendees completed a questionnaire prior to the formulation meeting and then another questionnaire following attendance at a formulation meeting.

Findings

The results indicate that staff found these to be a helpful, informative and a positive experience professionally and personally. The results suggest that the formulation meetings developed staffs’ psychological understanding about the patient and their problems, helped to increase their empathy towards the patient, increased consistency in the teams’ views, and that the staff felt listened to.

Research limitations/implications

Psychological formulation meetings are established in the current service, and therefore this may be a contributing to factor to the lack of significant change found in some of the items. Therefore, it would be beneficial for future services to complete a service evaluation at a much earlier point of implementation, as this may impact the level of significance.

Originality/value

The findings of this service evaluation suggest that formulation is a beneficial and useful tool for teams and would be a helpful tool for psychologists to use in the clinical work with teams.
