Abstract

Purpose

In 1999, the national health service (NHS) was made responsible for the commissioning of prison health care. Mental health inreach teams (MHIT) were set up to mirror community mental health teams and provide secondary care to prisoners diagnosed with severe and enduring mental illnesses (SEMI). Since then, the provision of mental health care to prisoners without a diagnosis of a SEMI has been variable. A rapid review of NHS health care in prisons conducted by Public Health England (PHE) (2016) highlighted the need for provision to be more integrated and meet the needs of prisoners without a diagnosis of a SEMI. In response, an integrated mental health and substance misuse service was implemented within her majesty’s prison/young offenders institution Pentonville. This study aims to evaluate its impact and share lessons learned.

Design/methodology/approach

Routinely collected and anonymised data were reviewed for prisoners referred between 1 May 2018 and 31 December 2019. Data are presented on the quantity of referrals over time, and the type of support offered. Chi-square goodness of fit tests was conducted to determine whether the prisoners referred to the service were representative of the wider prison population in terms of age and ethnicity.

Findings

Referrals showed a general pattern of increase over time and were representative of the wider prison population in terms of age and ethnicity, indicating equitable access. Lessons learned are discussed. Demand for therapeutic and substance misuse services was higher than that for SEMIs. Notable was the high quantity of referrals which provides further evidence for the disparity between high need and limited provision within prison settings, particularly for therapeutic interventions.

Originality/value

To the best of the author’s knowledge, this is the first service evaluation of a recently implemented integrated and holistic model of prison mental health care in line with recommendations from PHE (2016).

Abstract

Purpose

Prisons are uniquely challenging working environments. Staff are often exposed to direct and indirect trauma, impacting negatively on their mental well-being. Due to the limited research into prison staff experience, this paper aims to explore what staff find most challenging, how they cope, what support they would like and rewarding aspects of their work.

Design/methodology/approach

This service development project was facilitated through a staff well-being event. A qualitative approach was used and 74 staff members provided anonymised responses. An inductive and data-driven approach was used to analyse the data, and the trustworthiness of the analysis was considered using criteria established by Lincoln and Guba (1985).

Findings

Thematic analysis identified six themes, namely, the challenging nature of the work, interactions with prisoners, staff interactions, inadequate resources, staff support and development and coping strategies. Key findings include managing distress, self-harm and violence and limited resources presenting challenges. Role variety and opportunities to support prisoners were reported as positive. A variety of coping strategies were identified. Wider availability of supervision and reflective practice was suggested by staff.

Practical implications

Recommendations for increased staff support are made. Suggestions for future research investigating methods to increase rewarding aspects of work within prisons are given.

Originality/value

This analysis adds to the limited body of qualitative research investigating prison staff experiences; in particular, aspects of the work that they find rewarding such as the role variety and opportunities to make positive changes to prisoners’ lives. Novel coping strategies were identified, including cognitive reframing and behavioural strategies for managing stress, which could be encouraged to increase resilience.

Abstract

Purpose

This study examined organizational change in universities as it relates to discomfort among the organization's members.

Design/methodology/approach

Using the critical incident technique (CIT), data was collected from the informants in an Indonesian public university that had been mandated by the government to enter the top 500 world university ranking. This would make it a “World-Class” university.

Findings

The findings describe the causes, courses and consequences of the discomfort felt in response to the organizational change in the university context. The causes of discomfort were categorized as a fear of loss, organizational culture, systems and policies, work overload and a lack of resources. Discomfort can manifest through negative affective, cognition and behavioral tendencies. Meanwhile, the consequences result in active and passive participation in the process of the organizational change itself.

Originality/value

Discomfort with organizational change is a new variable that has rarely been explored, thus it requires testing and validation using different methods and contexts, as offered by this study. We have also shown that in the initial stage of organizational change (unfreezing), discomfort will always emerge that must be immediately managed in order not to trigger resistance to change. Furthermore, this study exhibits the use of the critical incident technique in the context of organizational change. Finally, we offer comprehensive views by exhibiting the causes, the reactions shown and the consequences of discomfort with the change.

Abstract

Purpose

The purpose of this paper is to develop a more detailed understanding of how embedding in different social networks relates to different types of action that individuals choose in the context of organizational closures, downsizing or relocations. To develop such insights, this paper focuses on three particular types of social networks, namely, intra-organizational; external professional and local community networks. These three types of networks have been frequently related to different types of action in the context of closures and relocations.

Design/methodology/approach

This is a conceptual paper. The authors develop the argument by integrating relevant recent literature on the salience related to embedding in different types of social networks, with a particular focus on responses to organizational closure or relocation.

Findings

The authors argue that at times of industrial decline and closure: embeddedness in intra-organizational networks can favor collective direct action; embeddedness in professional networks is likely to favor individual direct action and embeddedness in community networks can lead to individual indirect action. The authors then add nuance to the argument by considering a range of complicating factors that can constrain or enable the course (s) of action favored by particular combinations of network influences.

Originality/value

On a theoretical level, this paper adds to understandings of the role of network embeddedness in influencing individual and collective responses to such disruptive events; and direct or indirect forms of response. On a practical level, the authors contribute to understandings about how the employment landscape may evolve in regions affected by organizational demise, and how policymakers may study with or through network influences to develop more responsible downsizing approaches.

Abstract

Purpose

The purpose of this paper is to draw on data from 16 interviews (two each with eight women) to explore some of the ways in which everyday shopping may change as women become mothers. The meanings, practices and implications of the transition to motherhood have long been a topic for sociological inquiry. Recently, interest has turned to the opportunities offered by this transition for the adoption of more sustainable lifestyles. Becoming a mother is likely to lead to changes in a variety of aspects of everyday life such as travel, leisure, cooking and purchase of consumer goods, all of which have environmental implications. The environmental impacts associated with such changes are complex, and positive moves toward more sustainable activities in one sphere may be offset by less environmentally positive changes elsewhere.

Design/methodology/approach

This paper draws on data from 16 interviews (two each with eight women) to explore some of the ways in which everyday shopping may change as women become mothers.

Findings

This paper focuses on the ways in which modes and meanings of everyday shopping may shift through the transition to mother, and on indicating any potential sustainability implications. The paper explores the adoption of more structured shopping and of shifting the mode of grocery shopping online or offline. The paper draws attention to the way in which practices are embedded and interrelated and argue that more consideration needs to be given to the influence of all household members.

Originality/value

The question here is not whether women purchase different products or consume more once they have a child, but rather how does the everyday activity of shopping for groceries and the meanings it has change with new motherhood and what sustainability implications might this have? In this context, this paper provides a novel addition to research on new mothers and consumption.

Purpose

The purpose of this paper is to critically evaluate an innovative collaboration between health, housing and social care by exploring the “short stay project” apartments from service users’ perspectives and considering the effectiveness of this service model as part of enabling provision locally.

Design/methodology/approach

The qualitative methodology for this evaluation was interpretative phenomenological analysis (Smith, 2011), critically exploring service users’ personal lived experience of the “short stay project”. Three service users (n=3) participated in semi-structured interviews.

Findings

This study has identified the “short stay project” can prevent admission into and facilitate discharge from care and health services by offering a temporary stay in self-contained, adapted accommodation. Service users found value in staying at the apartments for differing reasons. However, practitioners must address service users’ emotional and social needs as well as physical needs to reduce the risk of occupational deprivation.

Research limitations/implications

Sample size is not fully representative of the total population making transferability limited.

Practical implications

This research found there is demand for temporary housing provision for service users with health, housing and/or social care needs.

Social implications

Key drivers of demand for the service are social inequalities relating to homelessness, poverty and gender-based violence rather than the health-related issues that could have been expected. Further research into the development of effective integrated services which maximise service users’ wellbeing and occupational performance is recommended.

Originality/value

Service models which integrate health, housing and social care can be innovative and maintain service users’ independence and wellbeing in the community. Commissioners across health, housing and social care could utilise the Better Care Fund to deliver integrated services to meet rising demands.

Abstract

Purpose

Many traditional retailers use the internet as a complementary business channel and thus sell products through multiple channels. In contrast, “pure player” internet retailers only sell products via the internet. A proliferation of pure players over the past few years has intensified competition in the online shopping market and the question of who is better at offering physical distribution service quality (PDSQ), multi‐channel retailers or pure players, is open to debate. Despite PDSQ's importance in an electronic commerce environment there are few studies to date and most have focused on general service quality of internet shopping. The purpose of this paper is to discuss this phenomenon and posit a conceptual framework for further investigation.

Design/methodology/approach

This paper reviews the literature to develop variables and constructs for investigation following the first stage of Churchill's paradigm for scale development, which are presented in a framework based on the concept of order fulfilment as a key driver in electronic PDSQ, or e‐PDSQ.

Research limitations/implications

The framework is presented for future investigation, thus there is no empirical study in this paper.

Practical implications

PDSQ has strategic importance for retailers to achieve competitive advantage and offer superior customer service, particularly for pure players as they are considered intangible services compared to their multi‐channel competitors. This importance also extends to the e‐commerce environment.

Originality/value

Earlier work has provided insight into how e‐PDSQ, represented by availability, timeliness and reliability, is affected by pricing, transactions and firms. This paper extends this work and presents an e‐PDSQ framework to investigate differences between multi‐channel and pure player retailers.

Abstract

Purpose

The purpose of this paper is to investigate the contextual factors contributing to the sustainability of healthcare quality improvement (QI) initiatives.

Design/methodology/approach

Themes from semi-structured interviews with international healthcare leaders are compared with Kaplan and Provost et al.’s (2012) model for understanding success in quality (MUSIQ). Critical success factors within these themes are shown in detail.

Findings

The interviews provide a rich source of information on critical success factors. The themes largely correspond with MUSIQ, reinforcing its robustness. An important factor emerging from the interviews was the importance of engagement with patients and families in QI, and this needs consideration in seeking to understand context in QI.

Research limitations/implications

Interview participants represent a limited set of western countries and health systems. Their experiences may not hold true in other settings.

Practical implications

The detail on critical success factors provides QI practitioners with guidance on designing and implementing sustainable initiatives.

Originality/value

Including consideration of contextual factors for engagement with patients and families in frameworks for context in QI appears to be an original idea that will add value to such frameworks. Researchers in patient engagement are starting to address contextual factors and connections should be made with this work.

Purpose

There are a limited number of risk assessment tools relevant to the internet child abuse material (I/CAM) offender cohort. The purpose of this paper is to introduce a new tool, the “Estimated Risk for Internet Child Sexual Offending” (ERICSO). The ERICSO has been developed to assist in the treatment and prevention of sexual crimes against children.

Design/methodology/approach

The ERICSO was developed through a multi-stage process including systematic review, survey of professionals and offender case study.

Findings

An empirically guided tool for estimating risk for I/CAM offenders, the ERICSO is composed of four domains. The Demographic domain questions provide information about the offender, while Collection domain questions address the content of the offender’s files. The Nature of Engagement domain considers the offender’s interaction with I/CAM, behavioural aspects and contact with children. The Social Aspect domain questions address the offender’s engagement with other I/CAM users. Finally, the assessor may use Structured Professional Judgement to provide additional information, and a summary of the offender’s relevant circumstances and a risk estimation.

Practical implications

The ERICSO provides guidance for the assessment of I/CAM offenders who may reoffend with online child sexual offences, and I/CAM offenders who may reoffend with contact child sexual offences.

Originality/value

Born from a history of generational attitudes towards rehabilitation and risk assessment, the launch of the ERICSO presents an exciting opportunity in the risk estimation of I/CAM offenders.

Abstract

Technology increasingly features in intimate relationships and is used by domestic violence perpetrators to enact harm. In this chapter, we propose a theoretical and practical framework for technology-facilitated harms in heterosexual relationships which we characterize as digital coercive control. Here, we include behaviors which can be classified as abuse and stalking and also individualized tactics which are less easy to categorize, but evoke fear and restrict the freedoms of a particular woman. Drawing on their knowledge of a victim/survivor's experiences and, in the context of patterns and dynamics of abuse, digital coercive control strategies are personalized by perpetrators and extend and exacerbate “real-world” violence.

Digital coercive control is unique because of its spacelessness and the ease, speed, and identity-shielding which technology affords. Victim/survivors describe how perpetrator use of technology creates a sense of omnipresence and omnipotence which can deter women from exiting violent relationships and weakens the (already tenuous) notion that abuse can be “escaped.” We contend that the ways that digital coercive control shifts temporal and geographic boundaries warrant attention. However, spatiality more broadly cannot be overlooked. The place and shape in which victim/survivors and perpetrators reside will shape both experiences of and response to violence. In this chapter, we explore these ideas, reporting on findings from a study on digital coercive control in regional, rural, and remote Australia. We adopt a feminist research methodology in regard to our ethos, research processes, analysis, and the outputs and outcomes of our project. Women's voices are foreground in this approach and the emphasis is on how research can be used to inform, guide, and develop responses to domestic violence.
