Abstract

Home from Hospital schemes run in the UK by the British Red Cross meet and match intermediate care policy's objectives. A review of 17 schemes undertaken in 1996 is compared with a review of 55 schemes operating in 2003. It suggests that such schemes are addressing needs and have the potential to develop work in prevention of hospital admission and to improve health generally.

Abstract

Local government and the NHS in Barking and Dagenham embarked on a bold initiative in 2001 to integrate health and social care management structures. Although it was not sustained, this local experience is an important source of learning as the search for improved partnership working enters yet another new phase. In particular, it demonstrates that the route to better outcomes depends on managing not only the tension between structure and culture, but also that between national targets and local discretion in services based on fundamentally different principles of governance: central management and local accountability.

Abstract

There has been a growing policy emphasis on prevention and the promotion of independence for older people. Are we clear, however, what we mean by prevention and how far authorities have been able to balance the needs of service users with promoting the well‐being of older people in general?

Abstract

National directives in the UK aim to enhance community provision for mentally ill people. One of these initiatives, the Care Programme Approach (CPA) requires health and social services to develop a needs‐led approach; care plans for discharged patients; co‐ordinated “packages” of health and social care and information systems. A survey conducted in the North Western Regional Health Authority area confirms many of the findings of recent work and raises other issues for health and social services managers. These include the need to: recognize and alter bad clinical‐organizational practices; develop assessment and priority rating systems which combine health and social care variables; implement relevant and accessible information systems; and develop adequate, standardized outcome measures for mental health services.
