Abstract

Purpose

The purpose of this paper is to present the findings of a study considering the application of the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS). Practitioners from a range of professions were recruited to provide their views of how to respond to a variety of scenarios. GPs, nurses, social workers, physio/occupational therapists and care assistants were recruited to participate.

Design/methodology/approach

This study used the Delphi method to elicit participant views and generate consensus of opinion. The Delphi method recommends a large sample for heterogeneous groups, and round one had 98 participants from six different professional groups.

Findings

Participants did not respond consistently to the scenarios, but disagreed most significantly when patient decisions conflicted with clinical advice, and when to conduct a capacity assessment. These responses suggest that clinical responses vary significantly between individuals (even within settings or professions), and that the application of Mental Capacity Act (MCA) is complicated and nuanced, requiring time for reflection to avoid paternalistic clinical interventions.

Originality/value

Previous studies have not used a Delphi method to consider the application of MCA/DoLS. Because of this methods focus on developing consensus, it is uniquely suited to considering this practice issue. As a result, these findings present more developed understanding of the complexity and challenges for practitioner responses to some relatively common clinical scenarios, suggesting the need for greater clarity for practitioners.

Abstract

Purpose

Perinatal mental health (PMH) is acknowledged as a significant public health issue associated with significant personal, family, social and economic burden. Research demonstrates that healthcare practitioners lack knowledge and confidence in this area but there is likely to be a complexity of factors that may influence practitioner behaviours, including negative attitudes towards people with mental health and inaccurate illness perceptions. The purpose of this paper is to evaluate the psychometric properties of the Perinatal Illness Perceptions Scale (PIPS), a conceptual derivation of the Illness Perception Questionnaire – Revised.

Design/methodology/approach

A cross-sectional and exploratory instrument development design, using exploratory factor analysis, was employed.

Findings

The scale demonstrated good psychometric properties revealing three sub-scales: causes, consequences (mother); consequences (baby).

Originality/value

The findings implicate the PIPS as the first robust psychometric measure, which can be used to in the assessment of practitioner knowledge of the causes and consequences of PMH. The PIPS could offer the opportunity to assess these domains within both educational and training context and identify practitioner attitudes which may affect clinical decision making and referral decisions.

Synopsis

Pebble Technology Corporation (Pebble) was an early entrant into the smartwatch industry. Pebble’s Founder, Eric Migicovsky, began thinking about creating a smartwatch in 2008 while still an undergraduate engineering student. After selling about 1,500 prototype watches, he was accepted into Silicon Valley’s prestigious Y Combinator business start-up program. Finding it difficult to attract investors, Migicovsky launched a crowdfunding campaign that raised a record-breaking $10.27m on Kickstarter. The case concludes shortly after Apple’s unveiling of its soon-to-be-released Apple Watch. The case provides an opportunity to evaluate Pebble’s various strategic options at the time of Apple’s announcement.

Research methodology

The authors observed over 30 h of video and audio recordings of speeches, interviews and other events involving Pebble’s founder, other Pebble executives, investors and competitors. These recordings are all publicly available. Whenever possible, the authors also reviewed the Twitter feeds, Facebook sites and personal websites of Pebble’s top executives over time. Similarly, the authors followed Pebble’s official website, corporate blog and Kickstarter campaign websites. The authors also drew from numerous media reports. Due to the public nature of the data, no company release is provided nor has any information been disguised in any way.

Case overview

This case examines the working environment of Fritz Publishing, a small independent South African publishing company. Fritz Publishing was established in 1960 by Nick Fritz. After his retirement, ownership passed to his son, Martin. In 2011, Martin Fritz decided to sell the company to the Prys Group, an international publishing house headquartered in Germany. February 2011 saw the arrival of a newly appointed CEO for Fritz Publishing, Vadim Arshavin, who had already experienced excellent financial results as the head of another publishing house. In the wake of his arrival, the company experienced several changes. The case highlights the challenges at Fritz Publishing that have resulted in a growing sense of dissatisfaction. After Martin Fritz sold Fritz Publishing, the organisational culture shifted quite drastically which created challenges for managers, employees and customers alike. Employees, including some members of management, are de-motivated, disengaged and frustrated because of the leadership style and behaviour of the new CEO Vadim Arshavin and consider their psychological contracts to have been breached. The case explores factors that have helped create this situation. It considers challenges to the sustainability of the organisation given recent events including an internal employee engagement survey and feedback from key customers. The case further examines the potential dangers that toxic leadership creates within organisations and encourages discussion on ways this form of destructive leadership can be handled.

Expected learning outcomes

The learning objectives to be drawn from the case are: to assess the impact of leadership on organisational culture; to analyse how leadership impacts the psychological contract; to identify the cross-cultural factors at play in an emerging market organisation and to understand the way a toxic leadership style can detrimentally affect a high-performance workplace. In addition, there are further learning objectives that can be explored. These are: to examine the change process and associated challenges with the introduction of new leadership into a family-type organisational culture; to understand how breach can be avoided and/or how the psychological contract can be reconstructed.

Abstract

The content of ethics education courses is still generally shaped around the presentation of the traditional ethical theories of Western moral philosophy, complemented by case studies and discussion of ethical decision-making models. The purpose of courses is still largely geared towards the development of skills in ethical reasoning. Yet developments in surrounding fields, from psychology to learning and leadership development, raise numerous questions about the traditional curriculum. Ethics courses need to be more responsive to psychological factors and to the social realities of workplace contexts, and cognisant of a wider spectrum of ethical concepts. The perspective of virtue ethics remains pertinent, as the broader agenda of ethics courses is to enable students to develop a personal ethical outlook. But ethics courses should also be exploring and incorporating concepts from non-Western philosophies, and incorporating developments in fields such as leadership development.

A contemporary review of the psychometric properties of the Buss‐Perry Aggression Questionnaire was undertaken to assess its suitability for an alcohol dependent population. Three criteria were used to try to achieve this: factor analysis; internal consistency reliability; and test‐retest reliability. Factor analysis revealed that its structure is remarkably consistent in a number of populations. Internal consistency reliability and test‐retest reliability results scores proved to be mainly above the recommended threshold. A conclusion was reached regarding these results that the Buss‐Perry Aggression Questionnaire is an effective screening tool for aggression in an alcohol dependent population.

Abstract

Purpose

The purpose of this paper is to explore how injecting opiate users on a methadone treatment programme experience stigma as drug addicts, and as service users in health care and pharmacy settings. In particular the paper explores the rationale for injecting drugs, which the paper is argued to create the conditions for experiencing shame at the micro interactional level, influenced by macro institutional factors. The paper links this issue of being an injecting drug user in treatment to question whether the definition of recovery as “drug free” in the Scottish drug policy document The Road to Recovery (2008) creates the potential for stigma of service users receiving methadone maintenance treatment.

Design/methodology/approach

In all, 14 participants, all of whom identified themselves as problem intravenous users of drugs, were recruited from three voluntary sector (third sector) treatment agencies in Scotland. Participants took part in semi-structured interviews; these were recorded, transcribed and qualitatively analysed thematically.

Findings

Participants describe feelings of stigma in relation to their drug taking as problem users. Their experiences as recovering opiate injectors raises further challenges in distancing themselves from stigmatised addict identities.

Originality/value

Reasons for injecting rather than smoking heroin were principally financially challenging a widely held belief that users inject primarily for pleasure, which is argued as increasing the potential for stigma. Shame and perceived discrimination was documented before and during drug treatment.

Abstract

Purpose

Reports on quality of life (QOL) measures for forensic patients are severely limited. The present paper aims to consider how to assess chronic patient's well‐being and to identify and evaluate the content validity of measures used to assess health‐related QOL in psychosis.

Design/methodology/approach

A review was undertaken to gather information on the different QOL measures from relevant databases, exploring their strengths and weaknesses.

Findings

Results from the review indicate a broad range of assessment tools are used in practice, although very few have been used in forensic settings. A preference for subjective tools is emerging, in addition to patient rated scales as opposed to clinician rated scales.

Originality/value

The application of QOL measures in the forensic population is of particular interest and a relatively new area of study, thus of value to practicing clinicians. It is hoped that the use of appropriate tools will enhance understanding of the treatment and service needs for mentally disordered offenders.
