Abstract

Purpose

Relational dynamics between patients and staff in forensic settings can be complicated and demanding for both sides. Reflective practice groups (RPGs) bring clinicians together to reflect on these dynamics. To date, evaluation of RPGs has lacked quantitative focus and a suitable quantitative tool. Therefore, a self-report tool was designed. This paper aims to pilot The Relational Aspects of CarE (TRACE) scale with clinicians in a high-secure hospital and investigate its psychometric properties.

Design/methodology/approach

A multi-professional sample of 80 clinicians were recruited, completing TRACE and attitudes to personality disorder questionnaire (APDQ). Exploratory factor analysis (EFA) determined factor structure and internal consistency of TRACE. A subset was selected to measure test–retest reliability. TRACE was cross-validated against the APDQ.

Findings

EFA found five factors underlying the 20 TRACE items: “awareness of common responses,” “discussing and normalising feelings;” “utilising feelings,” “wish to care” and “awareness of complicated affects.” This factor structure is complex, but items clustered logically to key areas originally used to generate items. Internal consistency (α = 0.66, 95% confidence interval (CI) = 0.55–0.76) demonstrated borderline acceptability. TRACE demonstrated good test–retest reliability (intra-class correlation = 0.94, 95% CI = 0.78–0.98) and face validity. TRACE indicated a slight negative correlation with APDQ. A larger data set is needed to substantiate these preliminary findings.

Practical implications

Early indications suggested TRACE was valid and reliable, suitable to measure the effectiveness of reflective practice.

Originality/value

The TRACE was a distinctive measure that filled a methodological gap in the literature.

Abstract

Purpose

This paper's aim is to explore factors underlying resilience to health adversity, where resilience is defined as better perceived health after adjusting for the presence of doctor diagnosed heart disease, stroke or diabetes (vascular disease).

Design/methodology/approach

A population sample of 667 men and women aged 50+ years from South Wales was recruited to participate in an epidemiologic study and were consented and assessed online. Participation included health status, psychological and cognitive assessment. Structural equation modelling was used to model causal pathways. The analysis presents baseline data for this sample.

Findings

After adjustment for vascular disease, self‐esteem was associated with higher perceived health (β=0.279, p<0.001) whilst depression was associated with lower perceived health (β=−0.368, p<0.001). Self‐efficacy and anxiety were not associated with perceived health. Further analysis found self‐esteem to buffer an effect of vascular disease on depression, reducing the impact of depression on perceived health.

Practical implications

Cognitive and affective factors are involved in resilience, in relation to health these are specific to self‐esteem and depression. Although more complex associations may be found with other adversities, in relation to health, interventions to improve self‐esteem and ameliorate depression are likely to increase resilience.

Originality/value

Resilience has been modelled as a process involving cognitive and affective response to adversity. In the context of health, the adverse effect of depression on health perception was mediated by self‐esteem. These associations add to the understanding of the processes underlying resilience and suggest opportunities for interventions designed to increase resilience to health adversities.

Abstract

Purpose

The study investigates the moderating effect of personal resources, including optimism and resilience, on the link between fear of Covid-19 and perceptions of academic safety among university students in Ghana.

Design/methodology/approach

A total of 618 students took part in the research by completing an online self-reported questionnaire. The respondents were chosen using a simple random sample method. The data was processed and analysed using IBM SPSS version 24 and SEM-PLS, respectively.

Findings

Results reveal fear of Covid-19 positively influence students' perception of academic safety. Furthermore, both resilience and optimism mitigate the impact of fear of Covid-19 on students' perceptions of academic safety.

Originality/value

This is the first study to examine personal resources as a moderator between fear of Covid-19 and students' perceptions of academic safety. Practical and theoretical implications are added to the text.

Abstract

Purpose

Young people who are looked after by the state face challenges as they make the transition from care to adulthood, with variation in support available. In the past decade, funding has been directed towards organisations to pilot innovations to support transition, with accompanying evaluations often conducted with a single disciplinary focus, in a context of short timescales and small budgets. Recognising the value and weight of the challenge involved in evaluation of innovations that aim to support the transitions of young people leaving care, this paper aims to provide a review of evaluation approaches and suggestions regarding how these might be developed.

Design/methodology/approach

As part of a wider research programme to improve understanding of the innovation process for young people leaving care, the authors conducted a scoping review of grey literature (publications which are not peer reviewed) focusing on evaluation of innovations in the UK over the past 10 years. The authors critiqued the evaluation approaches in each of the 22 reports they identified with an inter-disciplinary perspective, representing social care, public health and organisation science.

Findings

The authors identified challenges and opportunities for the development of evaluation approaches in three areas. Firstly, informed by social care, the authors suggest increased priority should be granted to participatory approaches to evaluation, within which involvement of young people leaving care should be central. Secondly, drawing on public health, there is potential for developing a common outcomes’ framework, including methods of data collection, analysis and reporting, which aid comparative analysis. Thirdly, application of theoretical frameworks from organisation science regarding the process of innovation can drive transferable lessons from local innovations to aid its spread.

Originality/value

By adopting the unique perspective of their multiple positions, the authors’ goal is to contribute to the development of evaluation approaches. Further, the authors hope to help identify innovations that work, enhance their spread, leverage resources and influence policy to support care leavers in their transitions to adulthood.

Abstract

Purpose

The purpose of this paper is to explore the issues and challenges facing employers as they manage degree apprentices in the workplace. It examines the relationship between managers and apprentices undertaking a work-based degree. This research is of particular relevance at this time because of the UK Government’s initiative to expand the number of apprenticeships in the workplace to three million new starts by 2020, inevitably bringing a range of pressures to bear on employers (BIS, 2015). The purpose is to share early experiences of employer management of degree apprenticeships, and provide a range of recommendations to develop and improve employer and higher education institution (HEI) practice.

Design/methodology/approach

This paper combines desk research with qualitative data drawn from interviews with a range of cross-sector organisations to investigate the employer’s experience of developing the new degree apprenticeships. Data are explored inductively using thematic analysis in order to surface dominant patterns and considers the implications of findings upon current and emerging HEI and employer practice and research.

Findings

There were a number of key themes which emerged from the data collected. These included the need for effective, employer-led recruitment processes, careful management of expectations, sound HEI retention strategies, employer involvement and board-level motivators to ensure organisational benefits are derived from effectively situated workplace learning and a focus upon effective, empowering mentoring and support strategies.

Research limitations/implications

As degree apprenticeship standards and programmes are currently at the early stages of implementation, and opportunities, funding and resourcing are rapidly changing in the context of government policy, so too will employer appetite and strategies for supporting degree apprentices, along with apprentice behaviour. This means that additional findings, beyond those highlighted within this paper, may emerge in the near future.

Practical implications

There are a number of practical implications supporting managerial development and support of degree apprentices in the workplace from this research. These are reflected in the findings, and include the development of flexible and collaborative processes, resources, mentor training and networks.

Originality/value

This paper is one of the first published accounts of the employers’ perspective of managing a degree apprenticeship within the new policy context in the UK. As a result, the work offers a unique insight into the emerging challenges and issues encountered by managers working with degree apprentices in the twenty-first century business environment.

Abstract

Purpose

The purpose of this paper is to use an assessment sheet clarifying the characteristics related to adaptation in order to support the relocation of older people to recovery phase rehabilitation wards (RPRW).

Design/methodology/approach

A survey was conducted with older people aged 70 and above two weeks after relocation. Nurses responsible for the older people were asked to complete the sheet. Of the 44 items on the sheet, the 38 items not directly related to basic attributes constituted the scale of the state of adaptation. A total of 336 valid response sheets were analyzed.

Findings

Higher scores on the scale represented higher levels of adaptation, and when scores were analyzed, results showed that there was a tendency for the scores of older people aged 80 and above and older people with dementia to be significantly lower. Items that showed differences according to the presence of dementia included those showing adaptive tasks, such as “Condition has deteriorated since the time of relocation” and “Experiencing difficulties when nurses are giving care.” The tendencies of older people to adapt to relocation depending on the presence of dementia were clarified through the items on the scale.

Originality/value

It is necessary to pay attention to older people's physical and social environmental factors, adaptive tasks, and coping skills when they relocate to RPRW. The authors suggest that an approach based on the results of this study could help older people adapt to their new environment.

Abstract

Purpose

The purpose of this paper is to engage stakeholders in the development of a community based chronic pain-management service and identify their different agendas for service design and delivery.

Design/methodology/approach

Data were collected using the Nominal Group Technique (NGT), a ten-step process that generates qualitative and quantitative data. Seven NGT groups were conducted in the south-west region of the UK, three with General Practitioners and nurses, three with chronic pain patients, and one with Healthcare Commissioners.

Findings

The patient agenda for service development focused on process of care issues particularly the need for deep- empathy and emotional support from providers, while professionals prioritised cost-effectiveness. While there was some overlap between agendas they were largely discrete and often contradictory.

Research limitations/implications

The findings imply service planners will need to make trade-offs between cost-containment and patient satisfaction. The methodology did not allow trade-offs to be put to participants in a structured form. However, such techniques are available, for example, Conjoint Analysis. There may also be value in bringing together patients and professionals in joint focus groups, to see if the gap between their different agendas can be bridged through discussion.

Originality/value

The findings provide a novel insight into the competing agendas of patients and professionals regarding service development and design which will be of value to service planners and managers as they strive to reconcile these differences.
