Abstract
Purpose
The Millfields unit is one of three medium secure hospital services on the Offender Personality Disorder (OPD) pathway, for the assessment and treatment of high-risk offenders who are likely to meet criteria for this diagnosis. This study has designed an audit to examine influencers of patient engagement within Millfields, whose treatment approach is that of an adapted therapeutic community (TC). The purpose of this study was to explore themes raised by patients in relation to engagement, drivers for and barriers to engagement and to make recommendations for improvement.
Design/methodology/approach
The first author engaged in ward and group observations, a review of each patient’s medical records, unstructured discussions with staff and semi-structured interviews with each patient exploring their experiences of therapy, relationships with staff, understanding of and engagement with the treatment model, attitudes towards peers and aspirations for the future. Themes were generated from the transcription of interviews and subsequent mind mapping.
Findings
In total, 12 of the 13 patients engaged in the semi-structured interviews. Six main themes were identified as being important influencers of engagement: the ward atmosphere, authenticity of engagement, perception of staff investment in the treatment, preferential treatment of senior residents, responses to boundary crosses and violations and the level of understanding a patient had of his treatment.
Originality/value
Research into engagement with psychosocial therapies in patients who have personality disorders has recently been published by Tetley et al. (2012) and Jinks et al. (2012). This study specifically focusses on TC engagement and male offenders with personality disorders.

Abstract
Women remain under-represented in leadership positions in both clinical medicine and medical education, despite a rapid increase in the proportion of women in the medical profession. This chapter explores potential reasons for this under-representation and how it can be ameliorated, drawing on a range of international literatures, theories and practices. We consider both the ‘demand’ for and ‘supply’ of women as leaders, by examining: how evolving theories of leadership help to explain women’s’ leadership roles and opportunities, how employment patterns theory and gender schemas help to explain women’s career choices, how women aspiring to leadership can be affected by the ‘glass ceiling’ and the ‘glass cliff’ and the importance of professional development and mentoring initiatives. We conclude that high-level national strategies will need to be reinforced by real shifts in culture and structures before women and men are equally valued for their leadership and followership contributions in medicine and medical education.

In this introductory chapter, I discuss the rationale for this edited collection and the contribution it can make to advancing knowledge of gender inequalities and promoting social justice in the medical profession and medical education. I provide a short overview of and critique of popular debates in the medical community in the United Kingdom and I also discuss briefly research about women’s careers in the United Kingdom and globally. The introductory chapter provides a description of each chapter and its contribution to scholarship about gender, careers and inequalities in Medicine/Medical Education.

Abstract
Purpose
Over 60% of armed conflicts re-occur; the seed of future conflict is sown even as a peace agreement is signed. The cyclical nature of war calls for a focus on youth who can disrupt this pattern over time. Addressing this concern, the developmental peace-building model calls for a dynamic, multi-level and longitudinal approach. Using an innovative statistical approach, this study aims to investigate the associations among four youth peace-building dimensions and quality peace.
Design/methodology/approach
Multi-level time-series network analysis of a data set containing 193 countries and spanning the years between 2011 and 2020 was performed. This statistical approach allows for complex modelling that can reveal new patterns of how different youth peace-building dimensions (i.e. education, engagement, information, inclusion), identified through rapid evidence assessment, promote quality peace over time. Such a methodology not only assesses between-country differences but also within-country change.
Findings
While the within-country contemporaneous network shows positive links for education, the temporal network shows significant lagged effects for all four dimensions on quality peace. The between-country network indicates significant direct effects of education and information, on average, and indirect effects of inclusion and engagement, on quality peace.
Originality/value
This approach demonstrates a novel application of multi-level time-series network analysis to explore the dynamic development of quality peace, capturing both stability and change. The analysis illustrates how youth peace-building dimensions impact quality peace in the macro-system globally. This investigation of quality peace thus illustrates that the science of peace does not necessitate violent conflict.

The objective of this discussion is to present an intersectional framework to better inform our reading and understanding of contemporary reports of sexual assault and sexual harassment. I posit that contemporary incidents of sexual violence must be read within the historical framework of slavery, where plantations served as the first site of sexual exploitation that has provided the ideological and practical scaffold for the continued erasure of the abuses of Black women and men in the workplace and under the law. This legacy, nonetheless, has yielded a coded language for according visibility to the “deep story” of rape and race in the United States.

Abstract
Around 7% of the female prison population are pregnant (Albertson, O'Keeffe, Lessing-Turner, Burke & Renfrew, 2014; Kennedy, Marshall, Parkinson, Delap, & Abbott, 2016; Prison Reform Trust, 2019). However, although recent years have witnessed growing academic interest in relation to mothering and imprisonment, limited attention has been paid to exploring the experiences of pregnancy for women serving a custodial sentence. Combining health and criminological research, this chapter offers a unique perspective of women's accounts of pregnancy and imprisonment, highlighting the specific challenges faced by pregnant women in negotiating the prison environment, whilst also illustrating the adaptive strategies adopted to cope with pregnancy and new motherhood in the context of imprisonment.
