Relationship marketing (RM) was conceived as an approach to industrial and service markets, and was considered inappropriate in other marketing contexts. Recently, however, the domain of RM has been extended to incorporate innovative applications in mass consumer markets. Much has changed in a few short years. Recent applications of RM in consumer markets have been facilitated by developments in direct and database marketing within an increasingly competitive and fragmented marketplace. This paper presents a critical review of the history of RM in consumer markets, and incorporates important conceptual, practical, empirical and popular contributions. A number of critical issues which remain unresolved are identified in the paper. These form the basis of ten research propositions which are crucial to justifying and advancing the domain extension into consumer markets.

Abstract
This chapter explores the emergence, growth, and current status of the sociology of sport in Canada. Such an endeavour includes acknowledging the work and efforts of Canadian scholars – whether Canadian by birth or naturalization or just as a result of their geographic location – who have contributed to the vibrant and robust academic discipline that is the sociology of sport in Canadian institutions coast-to-coast, and who have advanced the socio-cultural study of sport globally in substantial ways. This chapter does not provide an exhaustive description and analysis of the past and present states of the sociology of sport in Canada; in fact, it is important to note that an in-depth, critical and comprehensive analysis of our field in Canada is sorely lacking. Rather, this chapter aims to highlight the major historical drivers (both in terms of people and trends) of the field in Canada; provide a snapshot of the sociology of sport in Canada currently; and put forth some ideas as to future opportunities and challenges for the field in Canada.

Abstract
Purpose
Over the course of their service, veterans are exposed to elevated levels of chronic stress that contribute to a greater prevalence of mental illness than observed in the general population. When mental illness is present, comorbidity is normative. Convergent evidence suggests that co-occurring substance use and mental illness is among the most prevalent forms of comorbidity within veteran samples. The purpose of this paper is to explore issues associated with dual diagnoses among veterans in the USA.
Design/methodology/approach
Research on dual diagnoses among veterans was reviewed and consolidated for presentation into three substantive content areas consisting of prevalence, associated conditions, and treatment of dual diagnoses.
Findings
Dually diagnosed veterans represent a group at particularly high risk for myriad adverse biopsychosocial and treatment outcomes, including poor health, suicidality, violence or aggressive behavior, arrest, homelessness, and unemployment. A comprehensive strategy has been implemented within the Veterans Health Administration to address dual diagnosis and related problems. Additional research is required to more readily identify co-occurring substance use and mental illness and to refine integrated intervention approaches to minimize burden while improving treatment outcomes for veterans and their families.
Originality/value
The current review includes a wide range of research spanning more than two decades and describing dual diagnosis among combat veterans of all modern eras. Areas in need of further research (e.g. dual diagnosis among female veterans; early detection of psychopathology and fully integrated care among returning veterans) are identified and discussed.

Abstract
The series of articles on the reform of the British Museum Library, which began to appear, of all places in the world, in the columns of The Morning Post, on January 19th, 1912, should have the effect of informing the public on certain defects in the administration of that institution which are known to many who, unfortunately, remain silent. Among the alleged blemishes pointed out are the bad method of appointing the staff by a narrow and unsatisfactory system of nomination, which does not always succeed in procuring properly qualified men; the absence of a fairly complete subject catalogue; the tendency to give special attention to favourites among the readers by certain members of the staff; and the hopeless jumble of the classification in the reading room, and in the library generally. Other serious defects have been pointed out, such as the official insistence on limitations of various kinds both in the collecting and cataloguing of books; and, above all, the delays which occur in the delivery of requisitioned books to readers. The library is, bluntly speaking, a thoroughly stagnant place, which has only changed slightly for the better during the past thirty years. Although supported by the contributions of publishers, who are compelled to deposit copies of their publications, and by the people at large through the Civil Service Estimates of the Government, the library remains a kind of close corporation, swathed in red‐tape convention and official traditions to such an extent that it is of much use only to a handful of authors, antiquarian grubbers of various kinds, and the officials. It has done little for the man of business or the general student, or for the national education of the country, and the time has come when this great public library should be democratised and made useful and valuable to the many instead of to the pampered few. The army of effete literary hacks who haunt the reading room, without a perceptible infusion of fresh young life, and the lack of business‐like purpose in more than a third of the number, is probably unique. Why should such a group of ordinary individuals have practically the monopoly of a great publicly‐supported library, to the exclusion of the aspiring young, the practical man of affairs, and the citizens of the country in general? Why should such exclusion be brought about by impossible conditions, hours, regulations, and imperfect cataloguing, not to mention other defects? And why should a body of trustees (responsible to no one) and officials have power to perpetrate with impunity such injustice upon the general public of the country? These are questions which must now be threshed out in the open, and all professional men should applaud the Morning Post for its public‐spirited action.

Purpose
The purpose of this paper is to investigate whether, or to what extent, the audit society influences the professional context of Swedish pre‐hospital care.
Design/methodology/approach
Data were collected through an in‐depth case study of public supervision within ambulance services in Sweden. A three‐dimensional framework departing from three analytical concepts (knowledge base, formal organization and operational process) was used as a tool for further analysis of the field material.
Findings
The paper describes ambulance services under the pressure of frequent audits in a context where the knowledge base of ambulatory work was contested. While some have argued this would make it more receptive to an audit culture, the relational distance between the auditor and the auditees was high, which should indicate the opposite.
Originality/value
The paper explores the audit society and its processes. In this particular case, it could be argued that the impact of an audit culture on professional activities was moderated by the dissociated approach of the auditing team.
