Abstract

Purpose

Healthcare has undergone multiple phases in gaining understanding, accepting and implementing quality and safety, with the last 3 decades being crucial and decisive in making progress. During that time, safety has always been quoted along with quality, but the cost of error in healthcare (both in human lives and monetary cost) has been continuing to rise.

Design/methodology/approach

This article discusses the authors’ expert perspective in comparison to the industry’s research and practice outputs.

Findings

Healthcare has not yet defined quality and safety. This is allowing the misconception that already established quality management systems (QMSs) are fit for safety purposes as well. Even though aviation has acted as a paradigm for healthcare, further alignment in embedding safety management systems (SMS) has yet to be realised.

Originality/value

In this paper, the distinct nature of safety and its detachment of quality is being discussed, along with the need for clear and safety specific processes. Setting common language is the first step in establishing appropriate safety processes within SMSs, operating in tandem with QMSs, to promote patient safety successfully.

Abstract

Purpose

This viewpoint article discusses and analyses the need and benefits of a patient safety definition within the context of nursing.

Design/methodology/approach

This viewpoint article is supported by literature review, statutory documents and expert knowledge evidence. All these sources provided a unified narrative of the background, current aspects and future needs of patient safety.

Findings

The need for strengthening patient safety and the nurses' role within healthcare's actions towards patient safety are discussed. The predominant role of nurses due to the proportionate size and significant role along with the need for clarification of patient safety in nursing terms is recognised. Research evidence of nursing areas with safety issues and relevant nursing interventions are presented. Based on all findings, a research-based nursing specific patient safety definition is proposed. This definition includes three axes: what is patient harm, how this harm can be eliminated or reduced and which are the areas of nursing practice that are identified to provide opportunity for patient harm. These axes include nursing specifications of the patient safety definition.

Originality/value

It is the first time that a nurse specific patient safety definition is proposed. This definition strives to enhance nurse practitioners' understanding and engagement with patient safety by clarifying aspects of patient safety within everyday nursing practice.

Abstract

Purpose

Most military aviation organisations today have not evolved their safety management approach towards harmonising with civil aviation. Safety culture is the base for any civil aviation organisation, enabling employees to communicate effectively and be fully aware and extrovert on safety. Just culture and reporting culture both are related to safety culture. Both are parts of the awareness process, enhancing safety promotion. These distinct elements and the safety management systems (SMS) can serve well the military aviation. This paper aims to present and discuss the SMS philosophy, structure and elements as a solution for military aviation organisations.

Design/methodology/approach

The feature of civil aviation SMSs are presented and discussed, with reference to the applicable frameworks and regulations governing the SMS operation. A discussion on the challenges faced within the military aviation organisations, with a brief examination of a European Union military aviation organisation, is presented.

Findings

The European Military Airworthiness Requirements, which are based on the European Aviation Safety Agency set of rules, can act the basis for establishing military aviation SMSs. A civil-based approach, blended, as necessary, with military culture is workable, as this is the case for many defence forces that have adopted such aviation safety systems.

Originality/value

This viewpoint paper discusses the opportunities and challenges associated with the adoption of SMS by military aviation organisations. This is the first time that this issue is openly discussed and presented to the wider aviation community, outside military aviation.

Abstract

Purpose

The Airbus A320 family engine fan cowl doors (FCDs) safety issue is known to the industry for almost 18 years; however, it has not been addressed adequately by the aircraft manufacturer and the various operators and regulating authorities. The purpose of this paper is to examine in a systematic way the possible operational and safety implications of a new modification on the engine FCDs.

Design/methodology/approach

An array of error-prone scenarios is presented and analysed under the prism of human factors in a non-exhaustive qualitative scenario analysis.

Findings

All examined scenarios are considered more or less probable. A number of accident prevention solutions are proposed for each of the scenario examined, in view of the acceptance and implementation of this modification by operators.

Research limitations/implications

As these scenarios are neither exhaustive nor have been tested/validated in actual aircraft maintenance practice, the further analysis is necessary. A substantial follow-up survey should take place, which should include a wider array of scenarios. This would allow obtaining the necessary data for a quantitative (statistical) analysis.

Practical implications

This case study identifies issues in relation to this modification, introduced by Airbus and the European Aviation Safety Agency (EASA), which may prove problematic from the point of view of safety effectiveness and disruption of operations.

Originality/value

This case study examines a long-standing aviation safety issue and the implications of a solution proposed by the aircraft manufacturer and adopted by EASA. This can be useful in increasing the awareness around these issues and highlight the importance of a human-centric and scenario-based design of engineering modifications towards minimising error in aircraft technical operations.

Abstract

Purpose

Provisions for the minimisation of human error are essential through governance structures such as recruitment, human resource allocation and education/training. As predictors of safety attitudes/behaviours, employees’ personality traits (e.g. conscientiousness, sensation-seeking, agreeableness, etc.) have been examined in relation to human error and safety education.

Design/methodology/approach

This review aimed to explore research activity on the safety attitudes of healthcare staff and their relationship with the different types of personalities, compared to other complex and highly regulated industries. A scoping review was conducted on five electronic databases on all industrial/work areas from 2001 to July 2023. A total of 60 studies were included in this review.

Findings

Studies were categorised as driving/traffic and industrial to draw useful comparisons between healthcare. Certain employees’ personality traits were matched to positive and negative relationships with safety attitudes/behaviours. Results are proposed to be used as a baseline when conducting further relevant research in healthcare.

Research limitations/implications

Only two studies were identified in the healthcare sector.

Originality/value

The necessity for additional research in healthcare and for comparisons to other complex and highly regulated industries has been established. Safety will be enhanced through healthcare governance through personality-based recruitment, human resource allocation and education/training.

Abstract

Purpose

In this paper, the standard Peridynamic Timoshenko beam model accounting for the shear deformation is chosen to describe the thick beam kinematics. Unfortunately, when applied to very thin beam structures, the standard Peridynamics (PD) encounters the shear locking phenomenon, leading to incorrect solutions.

Design/methodology/approach

PD differs from classical continuum mechanics and other nonlocal theories that do not involve spatial derivatives of the displacement field. PD is based on the integral equation instead of differential equations to handle discontinuities and other singularities.

Findings

The shear locking can be successfully alleviated using the developed selective integration method. In particular, this technique has been implemented in the standard PD, which allows an accurate result for a wide range of slenderness from very thin to thick (10 < L/t < 103) structures. It can also accelerate the computational time for particular dynamic problems using fewer neighboring integration particles. Several numerical examples are solved to demonstrate the effectiveness of the proposed method for modeling beam structures.

Originality/value

The paper highlights the severe shear locking phenomenon in the Peridynamic Timoshenko beam available in the literature, especially for very thin structures. A new alternative for the alleviation of shear locking in the Peridynamic Timoshenko beam, using selective integration. Hence the developed Peridynamic Timoshenko beam model is effective for thin and thick structures. A new peridynamic formulation for the low-velocity impact beam models is presented and validated.
