Abstract
Purpose
Workflow optimisation is crucial for establishing a viable acute stroke (AS) intervention programme in a large tertiary care centre. This study aims to utilise Lean Six Sigma (LSS) principles to enhance the hospital's workflow.
Design/methodology/approach
The Action Research methodology was used to implement the project and develop the case study. The study took place in a large tertiary care academic hospital in India. The Define-Measure-Analyse-Improve-Control approach optimised the workflow within 6 months. Lean tools such as value stream mapping (VSM), waste audits and Gemba were utilised to identify issues involving various stakeholders in the workflow. Sigma-level calculations were used to compare baseline, improvement and sustainment status. Additionally, statistical techniques were effectively employed to draw meaningful inferences.
Findings
LSS tools and techniques can be effectively utilised in large tertiary care hospitals to optimise workflow through a structured approach. Sigma ratings of the processes showed substantial improvement, resulting in a five-fold increase in clinical outcomes. Specifically, there was a 43% improvement in outcome for patients who underwent acute stroke revascularisation. However, certain sigma ratings deteriorated during the control and extended control (sustainment) phases. This indicates that ensuring the sustainability of quality control interventions in healthcare is challenging and requires continuous auditing.
Research limitations/implications
The article presents a single case study deployed in a hospital in India. Thus, the generalisation of outcomes has a significant limitation. Also, the study encounters the challenge of not having a parallel control group, which is a common limitation in quality improvement studies in healthcare. Many studies in healthcare quality improvement, including this one, are limited by minimal data on long-term follow-up and the sustainability of achieved results.
Originality/value
This study pioneers the integration of LSS methodologies in a large Indian tertiary care hospital, specifically targeting AS intervention. It represents the first LSS case study applied in the stroke department of any hospital in India. Whilst most case studies discuss only the positive aspects, this article fills a critical gap by unearthing the challenges of applying LSS in a complex healthcare setting, offering insights into sustainable quality improvement and operational efficiency. This case study contributes to the theoretical understanding of LSS in healthcare. It showcases its real-world impact on patient outcomes and process optimisation.

Purpose
India liberalized its economy in 1991, which resulted in intense global competition, quality-conscious and demanding customers. Additionally, significant technological advancements lead to enhancements in products and processes. These forced Indian organizations to adopt innovative business strategies in the past 30 years. Meanwhile, the Lean Six Sigma methodology has significantly grown with vast applicability during the past 30 years. Thus, the purpose of this study is to develop the learning on Lean Six Sigma methodology in the Indian context through investigation of literature.
Design/methodology/approach
A three-stage systematic literature review approach was adopted to investigate the literature during the present study. In total, 187 articles published in 62 journals/conference proceedings from 2005 to 2022 (18 years) were shortlisted. The first part of the article summarizes the significant milestones towards the quality journey in the Indian context, along with the evolution of the Lean Six Sigma methodology. The second part examines the shortlisted papers on Lean Six Sigma frameworks, their applicability in industrial sectors, performance metrics, outcomes realized, publication trends, authorship patterns and leading researchers from the Indian perspective.
Findings
Lean Six Sigma has emerged as a highly acclaimed and structured business improvement strategy worldwide. The Indian economy has seen remarkable growth in the past decade and is one of the fastest-growing economies in the 21st century. Lean Six Sigma implementation in India has significantly increased from 2014 onward. The study revealed that researchers have proposed several different frameworks for Lean Six Sigma implementation, the majority of which are conceptual. Furthermore, the balanced applicability of Lean Six Sigma in manufacturing and service sectors was observed with the highest implementation in the health-care sector. Additionally, the widely adopted tools, techniques along with performance metrics exploring case studies were reported along with a summary of eminent and leading researchers in the Indian context.
Research limitations/implications
This study is confined to reviewed papers as per the research criteria with a significant focus on the Indian context and might have missed some papers due to the adopted papers selection strategy.
Originality/value
The present study is one of the initial attempts to investigate the literature published on Lean Six Sigma in the Indian context, including perspective on the Indian quality movement. Therefore, the present study will provide an understanding of Lean Six Sigma methodology in the Indian context to graduating students in engineering and management and entry-level executives. The analysis and findings on Lean Six Sigma frameworks, research approach, publications details, etc., will be helpful to potential research scholars and academia. Additionally, analysis of case studies on Lean Six Sigma implementation by Indian industries will assist the managers and professionals in decision making.

Abstract
Purpose
This article intends to comprehend the Lean Six Sigma (LSS) approach adopted in the aerospace industry in India for process improvement. This research has the objective of determining LSS deliverables such as Voice of Customer (VOC), Key Performance Indicators (KPIs), Critical-to-Quality (CTQ), project approach, deployment strategies and tools and techniques used to execute the project.
Design/methodology/approach
The study adopted an exploratory research methodology and a multiple case study analysis to draw robust inferences. The research is carried out in the Indian aerospace industry and analyses five case studies. The case studies were collated from the company via a case study protocol with pre-defined criteria. Also, a semi-structured interview is conducted with the stakeholders of each case study to determine the deployment strategies followed during the respective projects.
Findings
It is reconfirmed that LSS is crucial in the aerospace industry, particularly in engine and gear shops, to reduce rework and rejections. Also, it was found that cost and time savings are essential KPIs. Some LSS projects require multiple CTQs for process improvement in aero industries. The DMAIC approach is used for project execution, with the Design of Experiment (DOE) being an essential tool. Top management engagement, effective HRM practices, customer focus, cross-functional collaboration and clear roles are essential for successful LSS projects. Eventually, a road map was developed based on the analysis of multiple case studies.
Research limitations/implications
The study is focused on the aerospace industry in India, which may limit the generalizability of the findings to other industries or regions. The small sample size and reliance on qualitative data through semi-structured interviews may introduce subjectivity. Additionally, the long-term effects of LSS implementation, particularly in the context of evolving technologies, were not fully explored.
Practical implications
This study provides actionable insights for aerospace companies and related organisations to enhance quality and operational performance. The developed roadmap offers a practical guide for LSS deployment, helping organisations improve efficiency and competitiveness, especially in an era of economic slowdown and high competition.
Originality/value
The study reveals similarities and differences in LSS deliverables in Indian aerospace industries, creating a roadmap and tool matrix for project execution and serving as a template for manufacturing industries.

Purpose
The purpose of this paper is to propose and validate a robust industrial control system. The aim is to design a Multivariable Proportional Integral controller that accommodates multiple responses while considering the process's control and noise parameters. In addition, this paper intended to develop a multidisciplinary approach by combining computational science, control engineering and statistical methodologies to ensure a resilient process with the best use of available resources.
Design/methodology/approach
Taguchi's robust design methodology and multi-response optimisation approaches are adopted to meet the research aims. Two-Input-Two-Output transfer function model of the distillation column system is investigated. In designing the control system, the Steady State Gain Matrix and process factors such as time constant (t) and time delay (?) are also used. The unique methodology is implemented and validated using the pilot plant's distillation column. To determine the robustness of the proposed control system, a simulation study, statistical analysis and real-time experimentation are conducted. In addition, the outcomes are compared to different control algorithms.
Findings
Research indicates that integral control parameters (Ki) affect outputs substantially more than proportional control parameters (Kp). The results of this paper show that control and noise parameters must be considered to make the control system robust. In addition, Taguchi's approach, in conjunction with multi-response optimisation, ensures robust controller design with optimal use of resources. Eventually, this research shows that the best outcomes for all the performance indices are achieved when Kp11 = 1.6859, Kp12 = −2.061, Kp21 = 3.1846, Kp22 = −1.2176, Ki11 = 1.0628, Ki12 = −1.2989, Ki21 = 2.454 and Ki22 = −0.7676.
Originality/value
This paper provides a step-by-step strategy for designing and validating a multi-response control system that accommodates controllable and uncontrollable parameters (noise parameters). The methodology can be used in any industrial Multi-Input-Multi-Output system to ensure process robustness. In addition, this paper proposes a multidisciplinary approach to industrial controller design that academics and industry can refine and improve.

Purpose
Design science research (DSR) is a structured approach for solving complex ill-structured problems in organizations through the development of an artefact followed by its validation. This paper aims to evaluate existing DSR methodology and propose specific accents to promote DSR for environmental, social and governance (ESG)-oriented operational excellence (OPEX) initiatives within organizations.
Design/methodology/approach
This commentary paper is based on an abductive reasoning approach to evaluate and understand DSR and assess its effectiveness for developing solutions to typical ESG-oriented OPEX-based problems within organizations.
Findings
Existing literature on DSR is reviewed, after which it is evaluated on its ability to contribute to the implementation of sustainable solutions for ESG-oriented OPEX-based problems. Based on the review, specific DSR methodological accents are proposed for the development of ESG-oriented OPEX-based solutions in organizations.
Research limitations/implications
This conceptual paper contributes to the conceptual understanding of the applicability, limitations and contextual preconditions for applying DSR. This paper proposes an explicit and, in some ways, alternative view on DSR research for OPEX researchers to apply and further the body of knowledge on matters of sustainability (ESG) in operations management.
Practical implications
Currently, there is limited understanding and application of the DSR methodology for OPEX-based problem-solving initiatives, as appears in the scant literature on DSR applied for the implementation of OPEX based initiatives for ESG purposes. This paper aims to challenge and provide accents for DSR applied to OPEX-related problems by means of a DSR framework and thereby promotes intervention-based studies among researchers.
Originality/value
The proposed step-by-step methodology contains novel elements and is expected to be of help for OPEX-oriented academicians and practitioners in implementing DSR methodology for practical related problems which need research interventions from academics from Higher Education Institutions.

Purpose
This research aims to conduct a literature review of Lean Six Sigma (LSS) in the Indian context and related research publications and apply bibliometric analysis and the author's visualization to map research trends in this area.
Design/methodology/approach
This article conducts a bibliometric analysis of LSS-related research in the Indian context using data gathered from Scopus and Web of Science databases from 2011 to 2022. The review provides information on LSS-related research in the Indian context and evaluates performance based on primary sources, authors, keywords, countries, affiliations, and documents. The analysis employs the Biblioshiny app and Bibliometrix R-tool for data analysis and scientific mapping.
Findings
The results of the bibliometric analysis indicate that the LSS culture has widely spread in India. The International Journal of Lean Six Sigma and Production Planning and Control were found to be the most productive sources for publishing LSS-related research articles. Antony J. was identified as the most active author in this field, contributing the most over the years. Among all organizations, NITs have conducted the most comprehensive research on LSS, indicating their significant investment of resources and efforts in studying this methodology and its applications in India. Additionally, the study examined the intellectual, social, and conceptual structures to identify implicit gaps and future research opportunities.
Practical implications
The findings of this study can inform academicians, researchers, practitioners, and policymakers about the state-of-the-art and the specifics of the most prolific studies. This study will facilitate their exploration of emerging research areas in LSS.
Originality/value
To the best of the authors knowledge, this is the first bibliometric analysis of LSS in the Indian context, providing an overview of relevant publications published between 2011 and 2022. This study analyzed 194 articles on LSS in India, which can help researchers and academics identify emerging research areas, suitable collaborators, and relevant journals for future publications.

Purpose
New product development (NPD) is necessary for business sustenance and customer satisfaction. Six Sigma and Design for Lean Six Sigma (DLSS) efficiently employ the repetitive stages for NPD, leading to quality performance and profitability. This study aims to map the quality performance through NPD attributes through the Lean methodology.
Design/methodology/approach
The data on NPD were collected from 267 respondents from manufacturing companies to map the relationship between Six Sigma and DLSS for NPD. Confirmatory factor analysis was employed to confirm model fit, while structural equation modeling was employed to analyze the empirical data for framework testing. The study included nine variables and fourteen hypotheses identified from the literature.
Findings
The statistical results of this study show that NPD attributes such as innovation, marketing, organization, customer, product and technology positively influence the Lean Six Sigma structured improvement process (LSSSIP) and DLSS. Moreover, integrating these attributes in Lean planning enhance quality performance. This empirical investigation's findings indicate that ten of the 14 hypotheses were supported, giving the study a strong foundation.
Research limitations/implications
The data collection was limited to northern India; therefore, the results may not be generalizable to other areas of the world.
Practical implications
NPD involves handling technical issues and factors such as cost, operational bottlenecks, economic changes, competitors' strategy and company policy. This study helps understand the various NPD parameters and their relationship to Lean, which enables an effective NPD implementation strategy.
Originality/value
The current philosophy of NPD calls for a concurrent engineering approach; therefore, the entire organization must be part of this process. This study uses the holistic framework by optimizing NPD with Lean Six Sigma (LSS) principles. The study is unique in that, to date, research does not integrate NPD attributes with the objectives of LSS to develop an efficient NPD implementation strategy.

Purpose
The purpose of this study is to improve the magnetron quality in Company T by identifying the nonconforming defect, adjusting the factors affecting the leakage of the magnetron tube core, and determining the optimal parameter values of these factors.
Design/methodology/approach
A case study method is used to present the quality improvement of magnetron tube core. The define, measure, analyze, improve, and control framework is applied in the case study as well as several Six Sigma tools.
Findings
The results show that Ag–W thickness, Ag–W installation state and furnace entry interval are significant factors on the leakage of magnetron tube core, and the optimum settings for these factors are 0.055 mm, offset by 1 mm from the outer edge and 5 cm, respectively.
Research limitations/implications
The main limitation of this study is that it was carried out on a small number of production processes. The authors would like to analyze more case studies on the improvements of after-sales quality and supplier quality.
Practical implications
This research could be used in magnetron manufacturing process as a tool for managers and engineers to improve product quality, which can also be extended to similar manufacturing systems.
Originality/value
In this case study, the Six Sigma approach has been applied for the first time to solve magnetron manufacturing problems by improving the quality of magnetron production process. It can help the quality engineers be more familiar with the deployment of Six Sigma and effective tools.

This systematic literature review aims to synthesise, organise and structure the stock of knowledge relating to leadership and design for Lean Six Sigma (DFLSS) and to understand leadership’s role, traits, skills, competencies and styles within the context for DFLSS program success.
Design/methodology/approach
The research is based on a systematic literature review of 67 papers that were published on leadership and DFLSS, demonstrating the importance of leadership for successful DFLSS programs.
Findings
The key findings show that leadership plays a critical role in launching and sustaining a DFLSS program in organisations, while illustrating the leadership traits, skills, competencies and styles that are more conducive to a successful DFLSS program.
Originality/value
The study has identified several gaps in the literature from a practical approach and an empirical validation of the critical factors of leadership when applying and supporting DFLSS efforts supporting future initiatives.

Pursuing digital transformation is a valuable strategy to attain companies’ operational excellence and sustainable development. However, installing digital technologies and software is insufficient for achieving a successful digital transformation. Equally vital is providing digital solutions with reliable input data, which is a hard task in companies where information is gathered through manual or non-standardized processes. The lack of reliable data prevents technologies and software from operating at their best, hindering their ability to process information and derive correct insights for improvement. To avoid this, companies should embrace structured problem-solving approaches to evaluate current data retrieval processes, identify error sources and formulate countermeasures. This paper aims to provide an empirical study to substantiate A3 as a winning approach for advancing input data acquisition in companies.
Design/methodology/approach
A case study research is proposed, investigating the application of A3 in an ink manufacturing company, and checking how A3 improves data collection and company performance.
Findings
The case study corroborates A3 as an effective approach, allowing the removal of inefficiencies that previously went unnoticed and reaching a 10% productivity improvement.
Practical implications
