Abstract

This study examined the secular trends in socioeconomic inequality in obesity during the period 1971–1994 in the United States. We analyzed the national representative data collected from three waves of the National Health and Nutrition Examination Survey (NHANES) conducted between those years. The Concentration Index was calculated to measure the socioeconomic inequality in obesity across gender, age, and ethnic groups in each survey period. In general, socioeconomic inequality in obesity was reduced between the 1970s and 1990s in women and black men, although the trend was not statistically significant for black women and was stable in white men. Our results indicate that, first, the association between obesity and socioeconomic status (SES) weakened over time, and second, SES inequality was not an important contributor to the dramatic increase in the prevalence of obesity in the United States. Our findings suggest that other social and environmental factors, which have influenced changes in people’s lifestyle, might better explain the increasing overweight problem in the United States. Effective intervention efforts for the prevention and management of obesity should target all SES groups from a population perspective.

Abstract

Purpose

The purpose of this paper is to investigate how socioeconomic status and demographic factors were associated with child malnutrition as well as how these factors accounted for socioeconomic inequality of child malnutrition in Bangladesh during 2007-2011.

Design/methodology/approach

Data of this study come from two cross-sectional rounds (2007 and 2011) of the Bangladesh Demographic and Health Survey. This paper uses ordinary least square models to estimate the correlates of child malnutrition. The study further uses the concentration curve and the concentration index to estimate socioeconomic inequality of child malnutrition in Bangladesh. Finally, the decomposition method is used to explain socioeconomic inequality of child malnutrition in the study period.

Findings

Regression analysis indicates that child’s age, breast feeding, child’s birth order, the number of under-five child in the household, household wealth and parental education were strongly correlated with child malnutrition in Bangladesh. This study finds that absolute level of child malnourishment slightly decreased between 2007 and 2011, but socioeconomic inequality increased during this period. Children from the poorest household endured the burden of malnourishment more than those from the wealthiest households. The level of inequality also increased among the rural children, although it remained stagnant among the urban children. Decomposition analysis highlights that parental education had a significant negative relation with the average level of malnutrition, but its role was primarily centred among children from wealthier households.

Practical implications

An approach linking the ministry of health and education with other ministries may speed up the reduction of inequalities in social determinants of childhood undernourishment. Most importantly, there is a need for comprehensive government policies to reduce growing economic inequality and increase the relative income of the poor in Bangladesh.

Originality/value

This study is the first of its kind to apply the decomposition method to explain the socioeconomic inequality of child malnutrition in Bangladesh. This paper presents an enriched understanding of socioeconomic inequality of child malnutrition in Bangladesh during 2007-2011.

Abstract

Purpose

The purpose of this paper is to assess the trends of socioeconomic-related inequalities in maternal healthcare utilization in Ghana between 2003 and 2014 and examine the causes of inequalities in maternal healthcare utilization in Ghana.

Design/methodology/approach

Data are drawn from three rounds of the Ghana Demographic and Health Survey collected in 2003, 2008 and 2014, respectively. The authors employ two alternative measures of socioeconomic inequalities in health – the Wagstaff and Erreygers indices – to examine the trends of socioeconomic inequalities in maternal healthcare utilization. The authors proceed to decompose the causes of inequalities in maternal healthcare by applying a recently developed generalized decomposition technique based on recentered influence function regressions.

Findings

The study finds substantial pro-rich inequalities in maternal healthcare utilization in Ghana. The degree of inequalities has been decreasing since 2003. The elimination of user fees for maternal healthcare has contributed to achieving equity and inclusion in utilization. The decomposition analysis reveals significant contributions of individual, household and locational characteristics to inequalities in maternal healthcare. The authors find that educational attainment, urban residence and challenges with physical access to healthcare facilities increase the socioeconomic gap in maternal healthcare utilization.

Originality/value

There is a need to target vulnerable women who are unlikely to utilize maternal healthcare services. In addition to the elimination of user fees, there is a need to reduce inequalities in the distribution and quality of maternal health services to achieve universal coverage in Ghana.

Abstract

Purpose

A continuous mixed opinion on the relevance of caste-based reservations and caste as a factor of socioeconomic disparity in the recent period demands update of evidence on socioeconomic inequalities among caste groups for effective policy making. The purpose of this paper is to investigate whether the caste inequalities in terms of socioeconomic opportunities and poverty are still persisting in rural Uttar Pradesh based on village census surveys?

Design/methodology/approach

This study used data primarily collected from four village census surveys under the project rural transformation in Uttar Pradesh, 2013. Bivariate analyses, human opportunity index (HOI), multidimensional poverty index (MPI) and inequality decomposition analyses used as methods of analyses.

Findings

The authors findings suggest that in spite of more than six decades of welfare policies and major political mobilization movements among lower castes in the state, the huge inequalities in terms of critical socioeconomic indicators such as landholding, higher education and wealth distribution and multi-dimensional poverty across the castes are still persisting in the state. Decomposition results suggest that between group inequalities contribute more to the total inequality in landholding whereas within group inequalities contribute maximum to total inequality in education and wealth status of different castes in rural Uttar Pradesh. However, within inequalities much less in general castes compared to SCs/OBCs.

Originality/value

Based on its latest empirical evidence, this study strengthens the argument that caste still matters in socioeconomic achievements of the population in India even after decades of planning and financing of social welfare schemes to uplift the lower castes in India. Thus, provides critical inputs to current debates on the relevance of caste as a determinant of socioeconomic status in India.

Abstract

Recent worldwide gains in girls’ schooling are raising new questions about the continued relevance of gender for educational inequality. At issue is whether the time has come to shift the policy focus away from gender to socioeconomic status. Answers to this question, we suggest, depend on how gender gaps close, i.e., do they close irreversibly, evenly, and faster than socio-economic (SES)-related inequality?

Against this background and building on contrasted sociological perspectives on inequality, our chapter examines the recent convergence trajectories of several sub-Saharan countries, asking if these trajectories warrant a policy shift away from gender.

Our findings are mixed. Although, the magnitude of sex-related inequality in schooling is consistently smaller than SES-related inequality, the process of gender convergence remains reversible and it unfolds in top-down fashion. Such findings warrant continued attention to gender in sub-Saharan Africa, but with particular focus on poor girls and on synergies that address both female and poor children. This conclusion supports theoretical advances that transcend the Manichean divide between focus on cultural recognition and socioeconomic redistribution.

Abstract

We explore what health-capital theory has to offer in terms of informing and directing research into health inequality. We argue that economic theory can help in identifying mechanisms through which specific socioeconomic indicators and health interact. Our reading of the literature, and our own work, leads us to conclude that non-degenerate versions of the Grossman (1972a, 1972b) model and its extensions can explain many salient stylized facts on health inequalities. Yet, further development is required in at least two directions. First, a childhood phase needs to be incorporated, in recognition of the importance of childhood endowments and investments in the determination of later-life socioeconomic and health outcomes. Second, a unified theory of joint investment in skill (or human) capital and in health capital could provide a basis for a theory of the relationship between education and health.

Abstract

Purpose

This paper contributes to the literature by proposing an analysis of digital inequalities in Russia that focuses on two aspects hitherto under explored: the interregionality (by comparing and contrasting eight federal districts) and the multidimensionality of digital inequalities (by taking into account the three levels of digital divide). Therefore, the aim is to address the phenomenon of digital divide in Russia by discussing the three levels of the digital divide (access / skills / benefits) in a comparative and interregional perspective.

Design/methodology/approach

This paper uses secondary data for its analysis, including both national (e.g. the total number of daily Internet users in Russia) and more regionalized data (related to particular federal districts of Russia). The choice of data sources was determined by an attempt to provide a detailed and multifaceted coverage of all three levels of the digital divide in Russia, which is not limited to the access problem only. For this purpose, we are using and re-elaborating various reports about the development of the Internet and ICTs in Russia prepared by national and international organizations to cover the first level of the digital divide. To shed light upon the second and third levels of the digital divide, we discuss digital literacy report (2018), the report on Internet openness index of Russian regions (2017) and the report on the digital life index of the Russian regions (2016). Finally, in the attempt to map out the key directions of the state policy aimed at decreasing digital inequality in Russia, on both federal and regional levels, we analyze the most important regional and national policy measures to foster digitalization such as the digital Russia program, the digital government program and the program of eliminating digital inequality in Russia.

Findings

We consider this study to be both a first exploration and a baseline of the three level digital divides in Russia. The paper shows how the level of socioeconomic development of the federal districts, as well as a number of objective factors (distance/isolation, urbanization level, availability of infrastructure and costs for building new infrastructure, etc.) have impact upon digitalization of the regions. As a result, several federal districts of Russia (Central, Northwestern, and, in a number of cases, Ural and Volga federal districts) more often than others take leading positions in rankings, in terms of degree of Internet penetration, audience numbers, use of e-services, etc. This correlation, however, is not universal as we will show, and some regions lacking behind in terms of access can be booming in terms of digital literacy or other factors, like it happened with the Far Eastern Federal district for example. All in all, our research showed that digital inequality in Russia is still on place and will require more time for complete elimination, even though current state and public initiatives are being actively developed.

Originality/value

This paper will bring to light meaningful insights into the three levels of digital divides in Russia. Based on a multilevel (three levels of digital divide) and multi-sectional approach (the interplay of different types of inequalities), this paper contributed to overall better understanding of the digital inequalities phenomenon in Russia. It also allowed for a comparative interregional perspective, which has been missing in most papers on digital inequalities in Russia so far.

Abstract

Recent decades have witnessed a rising interest in the measurement of inequality from a multidimensional perspective. This literature has however remained largely theoretical. This chapter presents an empirical application of a recent methodology and in doing so offers practical insights on how multidimensional inequality can be measured over two attributes (wealth and health) in the developing country context. Following Abul Naga and Geoffard (2006), a methodological framework allowing the decomposition of multidimensional inequality into two univariate Atkinson–Kolm–Sen equality indices and a third term measuring the association between the attributes is implemented. The methodology is then illustrated using data from the World Health Surveys 2002–2003. Specifically, this study presents the first comparative analysis on multidimensional inequality for a set of Middle East and North African (MENA) countries. Results reveal that the multidimensional (in-)equality indices tend to mimic the (in-)equality ordering of the wealth distributions as the latter are always less equally distributed than health. An empirical conclusion that emerges is that reducing the correlation between the attributes may help to reduce overall welfare inequality, specifically when socioeconomic inequality in health is pro-poor. The finding that the correlation between attributes has a significant contribution in the quantification of inequality has important policy implications since it reveals that it is not only wealth and health inequalities per se that matter in the measurement of welfare inequality but also the associations between them.

Citizens are substantial stakeholders in every e-government system, thus their willingness to use and ability to access the system are critical. Unequal access and information and communication technology usage, which is known as digital divide, however has been identified as one of the major obstacles to the implementation of e-government system. As digital divide inhibits citizen’s acceptance to e-government, it should be overcome despite the lack of deep theoretical understanding on this issue. This research aimed to investigate the digital divide and its direct impact on e-government system success of local governments in Indonesia as well as indirect impact through the mediation role of trust. In order to get a comprehensive understanding of digital divide, this study introduced a new type of digital divide, the innovativeness divide.

The research problems were approached by applying two-stage sequential mixed method research approach comprising of both qualitative and quantitative studies. In the first phase, an initial research model was proposed based on a literature review. Semi-structured interview with 12 users of e-government systems was then conducted to explore and enhance this initial research model. Data collected in this phase were analyzed with a two-stage content analysis approach and the initial model was then amended based on the findings. As a result, a comprehensive research model with 16 hypotheses was proposed for examination in the second phase.

In the second phase, quantitative method was applied. A questionnaire was developed based on findings in the first phase. A pilot study was conducted to refine the questionnaire, which was then distributed in a national survey resulting in 237 useable responses. Data collected in this phase were analyzed using Partial Least Square based Structural Equation Modeling.

The results of quantitative analysis confirmed 13 hypotheses. All direct influences of the variables of digital divide on e-government system success were supported. The mediating effects of trust in e-government in the relationship between capability divide and e-government system success as well as in the relationship between innovativeness divide and e-government system success were supported, but was rejected in the relationship between access divide and e-government system success. Furthermore, the results supported the moderating effects of demographic variables of age, residential place, and education.

This research has both theoretical and practical contributions. The study contributes to the developments of literature on digital divide and e-government by providing a more comprehensive framework, and also to the implementation of e-government by local governments and the improvement of e-government Readiness Index of Indonesia.
